Situational strategic planning focusing on hypertension and diabetes in the family health strategy

Resumo: Introduction: Hiperdia consists in the registration of patients with Systemic Arterial Hypertension (SAH) and/or Type 2 Diabetes Mellitus (DM) attended by the Unified Health System. Moreover, it is known that the increase in life expectancy and the eating pattern of society contributes to the highlight of these comorbidities. SAH and Type 2 DM are predisposing factors for the emergence of other diseases, which together affect the quality of life of a large part of the socioeconomically vulnerable population. Objectives: To implement the Project in the Condor Family Health Strategy, aiming to reduce the problem of type 2 DM and SAH in the areas covered by the FHS and reduce the incidence through better prevention and health promotion. Methodology: It is a work in which problematization occurs by asking questions to the population about structural problems and recurrent diseases in the Basic Health Unit. Results: The action obtained satisfactory results that met the team's expectations. In addition, the participants of the action debated and discussed the theme addressed with the team, enabling the exchange of information and knowledge. Final considerations: Technical-scientific knowledge is of paramount importance, however the humanization of the professional must be the majority to understand the various realities and situations arising from the problems of patients in daily life, which was denoted in the methodology applied.

Además, se sabe que el aumento de la esperanza de vida y el patrón alimentario de la sociedade contribuyen al punto culminante de estas comorbilidades. SAH y Type2 DM son factores de predisposición para la aparición de otras enfermedades, que en conjunto afectan la calidad de vida de una gran parte de la población socioeconomicamente vulnerable. Objetivos: Implementarel Proyecto em la Estrategia de Salud Familiar, com el objetivo de reducir el problema de dm y SAH tipo 2 em las áreas cubiertas por el FHS y reducir la  incidencia a través de una mejor prevención y promoción de la salud. Metodologia: Este es um trabajo em el que la problematización se produce por preguntas hechas a la población sobre problemas estructurales y enfermidades recorrentes em la Unidad Básica de Salud. Resultados: La acción obtuvo resultados satisfactorios que cumplieron com las expectativas del equipo. Además, los participantes de la acción discutieron y discutieron el tema abordado com el equipo, permitiendo el intercambio de información y conocimientos. Consideraciones finales: El conocimiento técnico-científico es de suma importancia, sin embargo la humanización del profesional debe ser la mayoría para entender las diversas realidades y situaciones derivadas de los problemas de los pacientes em la vida diaria, que se denotó em la metodología aplicada.  Introdução: O Hiperdia consiste no cadastramento de portadores de Hipertensão Arterial Sistêmica (HAS) e/ou Diabetes Mellitus (DM) tipo 2 atendidos pelo Sistema Único de Saúde. Ademais, sabe-se que o aumento da expectativa de vida e o padrão alimentar da sociedade colaboram com o destaque dessas comorbidades. A HAS e DM tipo 2 são fatores de predisposição para o surgimento de outras doenças, que em conjunto afetam a qualidade de vida de boa parte da população vulnerável socioeconomicamente. Objetivo: Efetivar a aplicação do Projeto na Estratégia de Saúde da Família, objetivando reduzir a problemática da DM tipo 2 e HAS nas áreas cobertas pela ESF e reduzir a incidência através da melhor prevenção e promoção de saúde. Metodologia: Trata-se de um trabalho no qual a problematização ocorre por perguntas feitas à população sobre problemas estruturais e doenças recorrentes na Unidade Básica de Saúde. Resultados: A ação obteve resultados satisfatórios que atenderam às expectativas da equipe. Além disso, os participantes da ação debateram e discutiram o tema abordado com a equipe, possibilitando a troca de informações e conhecimento.
