Relationship between pain catastrophization and functional capacity of women with knee osteoarthritis: a cross-sectional study

Resumo: Knee osteoarthritis has pain as its main symptom. Catastrophizing is a cognitive factor that can influence painful sensations. This study aims to evaluate the presence of pain catastrophizing in women with knee osteoarthritis and the relationship of catastrophizing with functional capacity, pain, quality of life, and joint mobility. The sample consisted of women aged 50 to 80 years with knee osteoarthritis. The following were evaluated: Catastrophizing [Pain Catastrophizing Scale (PCS)]; Functional capacity [Tests: Chair-Stand; 40m walk; Stair-Climb]; Quality of life [Western Ontario and McMaster Universities Questionnaire (WOMAC) and Medical Outcomes Study 36 – Item Short-Form Health Survey (SF-36)] and knee joint mobility [goniometry]. According to the PCS result, the sample was divided into groups GCC (With catastrophizing) (n=10) and GSC (Without catastrophizing) (n=23). Mean and standard deviation analysis, Shapiro-Wilk test, Student's t-test or Mann–Whitney U test, and Spearman correlation were performed. It was considered p<0.05 and 95%CI. The sample consisted of 33 women, 30.30% had catastrophizing (PCS ≥ 30). The GCC had a lower score on WOMAC and a higher score on the SF-36 (p<0.05), except for the social aspects domain. The catastrophizing presented a significant correlation with the Sit-get up from a chair and Up-down stairs tests; with WOMAC and most SF-36 grades. The results suggest that catastrophizing is related to lower functional capacity, worse pain intensity, and quality of life in women with gonarthrosis.

El catastrofismo es un factor cognitivo que puede influir en la sensación dolorosa. El objetivo de este estudio fue evaluar la presencia de dolor catastrofizante en mujeres con artrosis de rodilla y la relación del catastrofismo con la capacidad funcional, el dolor, la calidad de vida y la movilidad articular. La muestra estuvo conformada por mujeres de 50 a 80 años con artrosis de rodilla. Se evaluaron: Catastrophizing [Pain Catastrophizing Scale (PCS)]; Capacidad funcional [Pruebas: Sentarse desde la silla; 40m a pie; Subir y bajar escalones]; Calidad de vida [Cuestionario de las Universidades de Western Ontario y McMaster (WOMAC) y Estudio de Resultados Médicos 36 – Encuesta de Salud de Formulario Corto (SF-36)] y movilidad de la articulación de la rodilla [goniometría]. De acuerdo con el resultado del PCS, la muestra se dividió en los grupos GCC (Con catastrofismo) (n=10) y GSC (Sin catastrofismo) (n=23). Se realizaron análisis de medias y desviación estándar, prueba de Shapiro-Wilk, prueba de la t de Student o prueba de la U de Mann-Whitney y correlación de Spearman. Se consideró p<0,05 e IC95%. La muestra estuvo constituida por 33 mujeres, el 30,30% presentaba catastrofismo (PCS ≥ 30). El GCC tuvo una puntuación más baja en WOMAC y una puntuación más alta en el SF-36 (p<0,05), excepto en el dominio de aspectos sociales. El catastrofismo presentó una correlación significativa con las pruebas Sentarse-levantarse de una silla y Subir-bajar escaleras; con WOMAC y la mayoría de los grados SF-36. Conforme o resultado da PCS, dividiu-se a amostra nos grupos GCC (Com catastrofização) (n=10) e GSC (Sem catastrofização) (n=23). Realizou-se análise de média e desvio-padrão, Teste de Shapiro-Wilk, Teste t de Student ou U de Mann–Whitney e Correlação de Spearman. Considerou-se p<0,05 e IC95%. A amostra foi composta por 33 mulheres, 30,30% apresentaram catastrofização (PCS ≥ 30). O GCC apresentou menor pontuação no WOMAC e maior escore no SF-36 (p<0,05), exceto no domínio aspectos sociais. A catastrofização apresentou correlação significativa com os testes de Sentar-levantar da cadeira e Subir-descer degraus; com o WOMAC e com a maioria das categorias do SF-36.
