Factors related to the high incidence of syphilis in pregnant women in Brazil: an integrative review

Resumo: Objective: To analyze the factors related to the high incidence of gestational and congenital syphilis in Brazil. Methodology: An integrative review of studies published between 2016 and 2021 was conducted through the LILACS, MEDLINE, PubMed and SciELO databases. Results: The main factors related to the high incidence of gestational and congenital syphilis, failure in diagnostic methods, underreporting, non-adherence to preventive and control methods, as well as the non- association of concomitant treatment of pregnant women and their partners were identified. In addition, there is a shortage of the raw material of benzathine penicillin, a first-line drug for treatment, and a deficiency in the qualification of health professionals who work in the face of treponemal infection in pregnant women and newborns. Conclusion: Treatment failure and increased incidence of gestational and congenital syphilis is directly related to the lack of training of health professionals, the non-treatment of the partner and the late start of prenatal care.

Metodología: Se realizó una revisión integradora de los estudios publicados entre 2016 y 2021 a través de las bases de datos LILACS, MEDLINE, PubMed y SciELO. Resultados: Se identificaron los principales factores relacionados con la alta incidencia de sífilis gestacional y congénita, el fracaso en los métodos de diagnóstico, la subnotificación, la no adhesión a los métodos preventivos y de control, así como la no asociación del tratamiento concomitante de las mujeres embarazadas y sus parejas. Además, hay escasez de la materia prima de la penicilina de benzatina, un medicamento de primera línea para el tratamiento, y una deficiencia en la calificación de los profesionales de la salud que trabajan frente a la infección treponémica en mujeres embarazadas y recién nacidos. Conclusión: El fracaso del tratamiento y el aumento de la incidencia de sífilis gestacional y congénita están directamente relacionados con la falta de formación de los profesionales de la salud, el no tratamiento de la pareja y el inicio tardío de la atención prenatal.Objetivo: Analisar os fatores relacionados a alta incidência da sífilis gestacional e congênita, no Brasil. Metodologia: Foi realizada uma revisão integrativa de estudos publicados entre os anos de 2016 à 2021, através das bases de dados LILACS, MEDLINE, PubMed e SciELO. Resultados: Foram identificados como principais fatores relacionados à alta incidência da sífilis gestacional e congênita, falha nos métodos diagnósticos, subnotificação, não adesão de métodos preventivos e de controle, bem como a não associação do tratamento concomitante das gestantes e seus parceiros. Além disso, nota-se uma escassez da matéria prima de penicilina benzatina, fármaco de primeira linha para o tratamento, e deficiência na qualificação dos profissionais de saúde que atuam diante da infecção treponêmica em gestantes e recém-nascidos.
