The primary care value chain of the supplementary system: an analysis of a private health plan

Resumo: | This paper aims to analyze the supply of primary health care (PHC) services in the supplementary health sector. The method employed was the application and elaboration of the Porter value chain (VC), based on a case study in a private health plan from the southern region of Brazil. It was possible to show that PHC services started to prioritize the Brazilian supplementary health, including regulation and incentive mechanisms by the National Supplementary Health Agency (ANS), due to its cost reduction potential. However, for the incorporation of PHC in the sector to occur sustainably, there is a need for an intense cultural change due to the solid medical tradition, which emphasizes the hospital model. More specifically, the main results were: i) PHC can add value to cooperative services, provided that investments are made in this technology; ii) the VC proved to be an adequate instrument to analyze the supply of PHC in supplementary health and could be used to outline strategies for operators to build their competitive advantage; and, iii) for the PHC plan analyzed, the generic strategy to be pursued would be to focus on differentiation. |

El método utilizado fue la aplicación y elaboración de la cadena de valor (CV) de Porter, a partir de un estudio de caso en un plan de salud privado de la región sur de Brasil. Fue posible evidenciar que los servicios de APS pasaron a priorizar la salud complementaria, incluyendo mecanismos de regulación e incentivos por parte de la Agencia Nacional de Salud Complementaria (ANS), dado su potencial para reducir costos. Sin embargo, para que la incorporación de la APS en el sector se dé de manera sostenible, es necesario un cambio cultural intenso, debido a la fuerte tradición médica existente en el sector. Más específicamente, los principales resultados fueron: i) APS puede agregar valor a los servicios de la organização, siempre y cuando se realicen inversiones en esta tecnología; ii) la CV demostró ser un instrumento adecuado para analizar la oferta de APS en salud complementaria, y puede ser utilizada para delinear estrategias para que los operadores construyan su ventaja competitiva; y, iii) para el plan APS analizado, la estrategia genérica a seguir sería el enfoque de diferenciación.Este trabalho tem o objetivo de analisar a oferta de serviços de Atenção Primária à Saúde (APS) no setor de saúde suplementar. O método empregado foi a aplicação e elaboração da cadeia de valor (CV) de Porter, a partir de um estudo de caso em um plano de saúde privado da região Sul do Brasil. Foi possível evidenciar que os serviços de APS passaram a ter prioridade pela saúde suplementar, incluindo mecanismos de regulação e de incentivos pela Agência Nacional de Saúde Suplementar (ANS), dado seu potencial de redução de custos. No entanto, para que a incorporação da APS no setor ocorra de forma sustentável, há necessidade de uma mudança cultural intensa, em virtude da forte tradição médica existente no setor.
