Importance of the central axes of the Brazilian Health Reform for the principles of the Unified Health System: reflections for the resistance to the neoliberal advance

Resumo: The objective of this study is to analyze the importance of building the principles of the Unified Health System (UHS) from the categories “Model of Social Determination” and “Process of Work in Health”, central axes of the Brazilian Health Reform (BHR). It is a reflective analysis based on a non-systematized literature review. Theoretical and conceptual frameworks were established in the light of the field of public health. The analysis was carried out in proximity to dialectical historical materialism, taking into account the assumptions of dynamicity and historicity. The development of the study is organized in: socio-historical analysis of health policies in Brazil until the mid-1980s; contextualization and conceptualization of the BHR Movement; and the analysis of the Model of Social Determination and the Process of Work in Health as central categories for the construction of SUS principles. Despite being brief, this study promoted reflection on the importance of building UHS principles from the central axes of BHR and Collective Health. It was possible to recover the historical paths taken to arrive at the philosophical, epistemological and ideological bases of the UHS, essential for the construction of a system based on the needs of the population and on non-alienation in the face of market rules. Thus, it becomes a powerful instrument for discussion in the interdisciplinary field of health and resistance to neoliberal advances, envisioning the scope of the UHS effectiveness.

Se trata de un análisis reflexivo basado en una revisión bibliográfica no sistematizada. Se establecieron marcos teóricos y conceptuales a la luz del campo de la salud pública. El análisis se realizó en proximidad al materialismo histórico dialéctico, teniendo en cuenta los supuestos de dinamicidad e historicidad. El desarrollo del estudio se organiza en: análisis sociohistórico de las políticas de salud en Brasil hasta mediados de la década de 1980; contextualización y conceptualización del Movimiento RBS; y el análisis del Modelo de Determinación Social y el Proceso de Trabajo en Salud como categorías centrales para la construcción de los principios del SUS. A pesar de ser breve, este estudio promovió la reflexión sobre la importancia de construir los principios del SUS a partir de los ejes centrales de la RBS y la Salud Colectiva. Fue posible recuperar los caminos históricos recorridos para llegar a las bases filosóficas, epistemológicas e ideológicas del SUS, esenciales para la construcción de un sistema basado en las necesidades de la población y en la no alienación frente a las reglas del mercado. Así, se convierte en un poderoso instrumento de discusión en el campo interdisciplinario de la salud y la resistencia a los avances neoliberales, vislumbrando el alcance de la efectividad del SUS.Objetivo deste estudo é analisar a importância da construção dos princípios do Sistema Único de Saúde (SUS) a partir das categoriais “Modelo de Determinação Social” e “processo de Trabalho em Saúde”, eixos centrais da Reforma Sanitária Brasileira (RSB). Trata-se de uma análise reflexiva a partir de uma revisão de literatura não sistematizada. Foram estabelecidos os marcos teóricos e conceituais sob a luz do campo da saúde coletiva. A análise foi conduzida em proximidade ao materialismo histórico dialético, atendendo aos pressupostos de dinamicidade e historicidade. O desenvolvimento do estudo está organizado em: análise socio histórica das políticas de saúde no Brasil até meados da década de 1980; contextualização e conceituação do Movimento da RSB; e a análise do Modelo de Determinação Social e do Processo de Trabalho em Saúde enquanto categorias centrais para a construção dos princípios do SUS. Apesar de breve, este estudo promoveu reflexão quanto a importância da construção dos princípios do SUS a partir dos eixos centrais da RSB e da Saúde Coletiva. Foi possível resgate histórico dos caminhos percorridos para se chegar às bases filosóficas, epistemológicas e ideológicas do SUS, imprescindíveis para a construção de um sistema baseado nas necessidades da população e na não-alienação frente às regras do mercado.
