Analysis of the avoidability of infant and fetal death in a medium-sized municipality in Brazil

Resumo: | Objective: To analyze the avoidability of infant and fetal deaths and the quality of research as indicators of iniquity in a medium-sized municipality in Brazil. Method: This is a documentary and quantitative study, with analysis of secondary data from the Mortality Information System (SIM) for the municipality of Caicó, in Rio Grande do Norte, reported in the period 2010 to 2015, with investigation of 58 infant deaths and fetal. SIM data were compared with data from the municipality's Mortality Committee based on the death certificate. Results: A low investigation of cases was identified (45.3%). Preventable deaths predominated (64.8%), evidenced by preventable deaths, if adequate attention was given to women during childbirth. There was a high degree of disagreement between the evaluation made in this study and the municipal investigation: fetal deaths, five cases (29.4%) presented agreement and, of infant deaths, only two (12.5%) Cases. Conclusion: There are weaknesses in the surveillance of death and in the performance of the municipal mortality committee. It is necessary to recognize the relevance of the investigation to the knowledge of these deaths as a tool to reduce health inequities. |

Método: Se trata de un estudio documental y cuantitativo, con análisis de datos secundarios del Sistema de Información de Mortalidad (SIM) para el municipio de Caicó, en Rio Grande do Norte, reportado en el período 2010 a 2015, con investigación de 58 muertes infantiles y fetales. Los datos del SIM se compararon con los del Comité de Mortalidad del municipio con base en el certificado de defunción. Resultados: Se identificó una investigación baja de los casos (45,3%). Predominaron las muertes prevenibles (64,8%), evidenciadas por muertes prevenibles, si se prestó la atención adecuada a las mujeres durante el parto. Hubo un alto grado de desacuerdo entre la evaluación realizada en este estudio y la investigación municipal: muertes fetales, cinco casos (29,4%) acuerdo y, de las muertes infantiles, sólo dos (12,5%) Casos. Conclusión: Hay debilidades en la vigilancia de la muerte y en el desempeño del comité municipal de mortalidad. Es necesario reconocer la pertinencia de la investigación para el conocimiento de estas muertes como herramienta para reducir las desigualdades sanitarias.Objetivo: Analisar a evitabilidade dos óbitos infantis e fetais e a qualidade da investigação enquanto indicadores de iniquidade em um município brasileiro de médio porte. Método: Trata-se de estudo documental e quantitativo, com análise de dados secundários do Sistema de Informações sobre Mortalidade (SIM) referente ao município de Caicó, no Rio Grande do Norte, notificados no período de 2010 a 2015, com investigação de 58 óbitos infantis e fetais. Foi feita uma comparação dos dados do SIM com dados do Comitê de Mortalidade do município a partir da declaração de óbito. Resultados: Foi identificado uma baixa investigação dos casos (45,3%). Os óbitos evitáveis predominaram (64,8%) sinalizando, provavelmente, inadequada atenção à saúde da mulher no parto. Verificou-se um alto grau de discordância entre a avaliação feita neste estudo e a investigação municipal: dos óbitos fetais, cinco casos (29,4%) apresentaram concordância e, dos óbitos infantis, somente dois (12,5%) casos. Conclusão: Existem fragilidades na vigilância do óbito e na atuação do comitê de mortalidade municipal.
