Adverse effects of prolonged use of non-steroidal anti-inflammatory drugs that inhibit COX-2 in horses: review

Resumo: Non-steroidal anti-inflammatory drugs (NSAIDs), being freely marketed, are among the most prescribed drugs in the world, have anti-inflammatory, analgesic and antipyretic action by inhibiting prostaglandins by blocking cycloxygenases (COX). Currently, it is known that there are three COX enzymes with different actions. It has been postulated that the pharmacological properties of NSAIDs stem mainly from the inhibitory action of COX-2, while adverse reactions are the result of COX-1 inhibition, creating subgroups of NSAIDs: preferential, selective and non-selective for COX-2. The introduction of COX-2 inhibitors in clinical practice aims to maintain anti-inflammatory efficacy without undesirable adverse effects. However, studies suggest toxicity associated with its use. The objective of this word is to gather information about the side effects of the prolonged use of NSAIDs that inhibit COX-2 in the equine species. For this, a bibliographic review was carried out looking for studies on the effects of the use of COX-2 inhibitors in horses. It was observed that this class of drugs has several toxic effects for the species, mainly in situations where there have been overdoses, prolonged use and association with other NSAIDs. Control measures are needed in the commercialization of these drugs. The choice of drug for treatment must be individualized for each patient, taking into account their risk factors and benefits. Its rational use is an important ally of veterinary clinicians in promoting animal health and well-being.

Actualmente, se sabe que hay tres enzimas COX con diferentes acciones. Se ha postulado que las propiedades farmacológicas de los AINE se derivan principalmente de la acción inhibitoria de COX-2, mientras que las reacciones adversas son el resultado de la inhibición de COX-1, creando subgrupos de AINE: preferenciales, selectivos y no selectivos para COX-2. La introducción de inhibidores de COX-2 en la práctica clínica tiene como objetivo mantener la eficacia antiinflamatoria sin efectos adversos indeseables. Sin embargo, los estudios sugieren toxicidad asociada con su uso. El objetivo de este trabajo es reunir información sobre los efectos secundarios del uso prolongado de AINE que inhiben la COX-2 en las especies equinas. Para ello, se realizó una revisión bibliográfica en busca de estudios sobre los efectos del uso de inhibidores de la COX-2 en caballos. Se observó que esta clase de drogas tiene varios efectos tóxicos para la especie, principalmente en situaciones donde ha habido sobredosis, uso prolongado y asociación con otros AINE. Se necesitan medidas de control en la comercialización de estos medicamentos. La elección del medicamento para el tratamiento debe ser individualizada para cada paciente, teniendo en cuenta sus factores de riesgo y beneficios. Su uso racional es un aliado importante de los médicos veterinarios para promover la salud y el bienestar de los animales.Os antiinflamatórios não esteroidais (AINE's), por serem comercializados, suprimidos-se entre os medicamentos mais prescritos no mundo, possuindo ação anti-inflamatória, analgésica e antipirética por inibição das prostaglandinas mediante bloqueio das cicloxigenases (COX). Atualmente sabe-se da existência de três enzimas COX com ações distintas. Postulou-se que as propriedades farmacológicas dos AINE's decorrem principalmente da ação inibitória da COX-2, enquanto que as reações adversas são geradas da inibição da COX-1, criando subgrupos de AINE's: preferenciais, seletivos e não seletivos para COX-2. A introdução dos inibidores da COX-2 na prática clínica visa manter a eficácia antiinflamatória sem os efeitos adversos indesejáveis. Todavia, estudos comprovados toxidade associada ao seu uso. Objetivou-se com este trabalho levantar informações sobre os efeitos do uso prolongado de inibidores da COX-2 na espécie de AINE. Para isso, foi realizada uma revisão bibliográfica buscando estudos sobre os efeitos do uso de inibidores da COX-2 em cavalos. Foi observado que esta classe de medicamentos diversos efeitos tóxicos para uma espécie, principalmente em situações onde existiram sobredosagens, uso por tempo prolongado e associação com outros AINE's. São necessárias para o controle de comercialização desses fármacos. A escolha do fármaco no tratamento deve ser individualizada a cada paciente, levando em consideração seus fatores de risco e benefícios.
