Hypertension in the Elderly: determining factors for non-adherence to drug therapy, potentiated by the COVID pandemic 19

Resumo: Objective: To discuss and highlight in the literature the determining factors of non-adherence of elderly patients with RAS to drug therapy, potentiated by the COVID pandemic 19. Material and method: This is a literature review that aims at the analysis and synthesis of the determining factors for the non-adhering of the elderly with hypertension to drug treatment. We analyzed the publications produced between 2017 and 2022, with full texts and available in the Scientific Electronic Library Online (SciELO), Latin American and Caribbean Literature in Health Sciences (LILACS) and Google Scholar, in addition to the manual of the 7th Brazilian Guidelines on Arterial Hypertension and the World Health Organization (WHO). Results: The search in the databases resulted in the identification of 1,203 articles. After the final reading, 32 articles were selected that contemplated the objectives discussed in the development of this work. Final considerations: It was found that many factors interfere with the non-adherence of the elderly to drug therapy, factors that are linked to the elderly and the health system, as the patient presents difficulties in following the proposed therapeutic scheme, either due to its complexity, forgetfulness, adverse effect of the drug and its belief in alternative empirical drugs.

Material y método: Se trata de una revisión de la literatura que tiene como objetivo el análisis y síntesis de los factores determinantes para la no adhesión de los ancianos con hipertensión al tratamiento farmacológico. Se analizaron las publicaciones producidas entre 2017 y 2022, con textos completos y disponibles en la Scientific Electronic Library Online (SciELO), Latin American and Caribbean Literature in Health Sciences (LILACS) y Google Scholar, además del manual de las 7ª Directrices Brasileñas sobre Hipertensión Arterial y la Organización Mundial de la Salud (OMS). Resultados: La búsqueda en las bases de datos resultó en la identificación de 1.203 artículos. Después de la lectura final, se seleccionaron 32 artículos que contemplaban los objetivos discutidos en el desarrollo de este trabajo.  Consideraciones finales: Se encontró que muchos factores interfieren con la no adhesión de los ancianos a la terapia medicamentosa, factores que están vinculados a los ancianos y al sistema de salud, ya que el paciente presenta dificultades para seguir el esquema terapéutico propuesto, ya sea por su complejidad, olvido, efecto adverso del fármaco y su creencia en fármacos empíricos alternativos.Objetivo: Discorrer e evidenciar na literatura os fatores determinantes da não adesão do paciente idoso portador de HAS à terapêutica medicamentosa, potencializados pela pandemia de COVID 19. Material e método: Trata-se de uma revisão de literatura que tem como finalidade a análise e síntese dos fatores determinantes para a não adesão do idoso portador de hipertensão arterial ao tratamento medicamentoso. Foram analisadas as publicações produzidas entre 2017 e 2022, com textos completos e disponíveis nas bases de dados Scientific Eletronic Library Online (SciELO), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e Google Acadêmico, além do manual da 7ª Diretrizes Brasileira de Hipertensão Arterial e da Organização Mundial da Saúde (OMS). Resultados: A busca nas bases de dados resultou na identificação de 1.203 artigos.
