Extraction associated with L-PRF grafting and immediate installation - Case reports

Resumo: Within the regenerative procedures, fibrin rich in platelets and leukocytes (L-PRF) is discussed. The process for obtaining an L-PRF is considered simple, it starts with a venipuncture through the patient, where the use of anticoagulants is dispensed with. Considered as a second-generation platelet biomaterial, it is a means of concentrated and slow release of growth factors, enhancing bone neoformation. The objective of this work is to discuss, through 6 clinical case reports, the use of fibrin rich in platelets and leukocytes used after dental extraction procedures associated with the immediate installation of osseointegrated implants and its contribution to the success of the treatment. Bone gaps were compared at the time of extraction with implant placement and after the period of osseointegration, revealing total bone neoformation and osseointegration in the 6 cases presented. The bone types found varied from type I to IV, in the previously performed grafting sites. Therefore, the extraction technique associated with the installation of osseointegrated implants with the L-PRF and PFR-Block grafting technique is suggested to be safe, enabling bone neoformation and osseointegration.

El proceso para la obtención de una L-PRF se considera simple, se inicia con una venopunción a través del paciente, donde se prescinde del uso de anticoagulantes. Considerado como un biomaterial plaquetario de segunda generación, es un medio de liberación concentrada y lenta de factores de crecimiento, potenciando la neoformación ósea. El objetivo de este trabajo es discutir, a través de 6 casos clínicos, el uso de fibrina rica en plaquetas y leucocitos después de procedimientos de extracción dental asociados a la instalación inmediata de implantes osteointegrados y su contribución al éxito del tratamiento. Se compararon los espacios óseos en el momento de la extracción con la colocación del implante y después del período de osteointegración, revelando neoformación ósea total y osteointegración en los 6 casos presentados. Los tipos de hueso encontrados variaron de tipo I a IV, en los sitios de injerto realizados previamente. Por tanto, la técnica de extracción asociada a la instalación de implantes osteointegrados con la técnica de injerto L-PRF y PFR-Block sugiere ser segura, permitiendo la neoformación y osteointegración ósea.Dentro dos procedimentos regenerativos discute-se a fibrina rica em plaquetas e leucócitos (L-PRF). O processo para a obtenção de uma L-PRF é considerado algo simples, sendo obtida através da venopunção do próprio paciente. Considerado como um biomaterial plaquetário de segunda geração, é um enxerto plaquetário de liberação concentrada e lenta de fatores de crescimento, potencializando a neoformação óssea. O objetivo deste trabalho foi de discutir através de 6 relatos de casos clínicos o uso da fibrina rica em plaquetas e leucócitos, empregada após os procedimentos de extração dentária associada à instalação imediata de implantes osseointegrados e sua contribuição para o sucesso do tratamento. Foram comparados os gaps ósseos no momento da extração com a instalação do implante e após o período de osseointegração, revelando total neoformação óssea e osseointegração dos 6 casos apresentados. Os tipos ósseos encontrados variaram de tipo I a IV, nos locais de enxertia anteriormente realizadas.
