Analysis of infections caused by carbapenemase-producing gram-negative bacteria in a hospital in the Campos Gerais region

Resumo: Objective: To determine the profile and prevalence of hospital infections caused by carbapenem-resistant Gram-negative bacteria related to healthcare. Methodology: This retrospective cross-sectional descriptive study (Yin, 2015) analyzed the profile of infections in a single public hospital by collecting data from electronic medical records. The collection included demographic data and infection-related clinical and laboratory findings. Results: We analyzed 193 acquisitions of Gram negative bacteria, 25 of which were by carbapenem-resistant microorganisms. Of these, the most common species detected was Klebsiella pneumoniae (56%, 14/25).  As for infections, the predominant hospital sector was the intensive care unit (56%, 14/25), mostly related to ventilator-associated pneumonia (40%, 10/25) and in patients submitted to some invasive procedure (72%, 18/25). About 50% of the patients had some adjacent health condition. Antimicrobial treatment prior to diagnosis was identified in 72% (19/25) of patients, with a predominance of betalactams (85.7%, 12/19) and after diagnosis, the majority used monotherapy (72%, 19/25). The impact of the infection was that 68% (17/25) were admitted to the intensive care unit, 40% (10/25) died, and 60% (15/25) were discharged. Conclusion: The results are in agreement with current literature and highlight the importance of prevention in combating the spread of these microorganisms in hospital environments.

Metodología: Se trata de un estudio descriptivo transversal retrospectivo (Yin, 2015) y se analiza el perfil de las infecciones en un único hospital público, mediante la recogida de datos en las historias clínicas electrónicas. La recopilación incluyó datos demográficos y hallazgos clínicos y de laboratorio relacionados con la infección. Resultados: Se analizaron 193 adquisiciones de bacterias Gram negativas, de las cuales 25 fueron por microorganismos resistentes a los carbapenemes. De ellas, la especie más común detectada fue Klebsiella pneumoniae (56%, 14/25).  En cuanto a las infecciones, la unidad de cuidados intensivos fue el sector hospitalario predominante (56%, 14/25), más relacionadas con la neumonía asociada a la ventilación (40%, 10/25) y en pacientes sometidos a algún procedimiento invasivo (72%, 18/25). Alrededor del 50% de los pacientes tenían alguna condición de salud adyacente. El tratamiento antimicrobiano antes del diagnóstico se identificó en el 72% (19/25) de los pacientes, con un predominio de los betalactámicos (85,7%, 12/19) y después del diagnóstico, la mayoría utilizó monoterapia (72%, 19/25). El impacto de la infección fue que el 68% (17/25) fueron ingresados en la unidad de cuidados intensivos, el 40% (10/25) evolucionaron hasta la muerte y el 60% (15/25) fueron dados de alta.  Conclusión: Los resultados están en concordancia con la literatura actual y destacan la importancia de la prevención para combatir la propagación de estos microorganismos en los ambientes hospitalarios.Objetivo: Determinar o perfil e a prevalência das infecções hospitalares causadas por bactérias Gram negativas resistentes aos carbapenêmicos relacionadas à saúde. Destes, a espécie mais comum detectada foi a Klebsiella pneumoniae (56%, 14/25).  Quanto as infecções, o setor hospitalar predominante foi a unidade de terapia intensiva (56%, 14/25), a maioria relacionado a pneumonia associada a ventilação mecânica (40%, 10/25) e em pacientes submetidos a algum procedimento invasivo (72%, 18/25). Cerca de 50% dos pacientes possuía alguma condição de saúde adjacente. O tratamento antimicrobianos anterior ao diagnóstico foi identificado em 72% (19/25) dos pacientes, com predomínio de betalactâmicos (85,7%, 12/19) e após o diagnóstico, a maioria utilizou monoterapia (72%, 19/25).
