Profile of systemic antifungal use in an Intensive Care Unit of a high complexity hospital

Resumo: In recent decades, there has been a considerable increase in the incidence and diversity of fungal infection. As a result, 1.5 million patients die annually in hospitals due to invasive fungal infections. This article aims to evaluate the profile of antifungal use in adults in the ICU of a University Hospital. This is a cross-sectional and retrospective study carried out in the ICU of the Getúlio Vargas University Hospital, located in the State of Amazonas, belonging to the Unified Health System (SUS), from March/2019 to May/2021. A total of 100 patients were included in the study, the mean age was 53.86 ± 16.11 years, and the mean length of stay in the hospital was 41.08 ± 29.09 days. The main indication for antifungal therapy was the treatment of candida infection, followed by respiratory tract infections. Among the antifungal treatments prescribed, micafungin stood out. When analyzing consumption by DDD, it was observed that there had been a gradual increase in the use of micafungin in recent years. That way, it is concluded that the complications resulting from the infection by COVID-19 changed the profile of use of this important class. In this context, the CCIH is one of pillars for establishing adequate criteria in the choice of antifungal therapy.

El objetivo de este artículo es evaluar el perfil de uso de antifúngicos en adultos en la UCI de un Hospital Universitario. Se trata de un estudio transversal y retrospectivo realizado en la UTI del Hospital Universitário Getúlio Vargas, ubicado en el Estado de Amazonas, perteneciente al Sistema Único de Salud (SUS), de marzo/2019 a mayo/2021. Se incluyeron en el estudio un total de 100 pacientes, la edad media fue de 53,86 ± 16,11 años y la estancia media hospitalaria fue de 41,08 ± 29,09 días. La principal indicación para la terapia antifúngica fue el tratamiento de la infección por cándida, seguida de las infecciones del tracto respiratorio. Entre los tratamientos antifúngicos prescritos destacaba la micafungina. Al analizar el consumo por DDD se observó que en los últimos años se ha producido un incremento paulatino en el uso de micafungina. Sin embargo, se concluye que las complicaciones derivadas de la infección por COVID-19 cambiaron cualitativamente el perfil de uso de esta importante clase. En este contexto, el CCIH representa una de las bases para el establecimiento de criterios adecuados en la elección de la terapia antifúngica.Nas últimas décadas, houve um aumento considerável na incidência e diversidade de infecções fúngicas, estima-se que 1,5 milhões de pacientes morrem anualmente no âmbito hospitalar em decorrência das infecções fúngicas invasivas. O objetivo deste artigo é avaliar o perfil de utilização de antifúngicos em adultos na UTI de um Hospital Universitário. Trata-se de um estudo transversal e retrospectivo realizado na UTI do Hospital Universitário Getúlio Vargas, localizado no Estado do Amazonas, pertencente ao Sistema Único de Saúde (SUS), no período de março/2019 a maio/2021. Ao total cem pacientes foram incluídos no estudo, a média de idade foi de 53,86 ± 16,11 anos e o tempo médio de permanência na unidade hospitalar foi de 41,08 ± 29,09 dias. A principal indicação para terapia antifúngica foi tratamento de infecção por cândida, seguida de infecções do trato respiratório. Dentre os tratamentos antifúngicos prescritos destacou-se a micafungina. Quando analisada o consumo por DDD observou-se que nos últimos anos ocorreu um aumento gradativo na utilização de micafungina. Nesse sentido, conclui-se que as complicações advindas da infecção por COVID-19 alteraram o perfil de utilização dessa importante classe.
