Hardships faced by the trans population in the access to health services: an integrative literature review

Resumo: The access to health services is a constitutional right of all people. This article aimed to carry out an integrative review about the difficulties faced by the trans population to access health services. 15 articles, published from 2012 to 2021, were found in the databases Índice Bibliográfico Español en Ciencias de la Salud (IBECS), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), and Scientific Electronic Library Online (SciELO). To discuss the results, four categories of analysis were created: Overview of Legislation Milestones on the Rights of the Trans Population; Stigma and Discrimination Against the Trans Population; The Binary and Biological Logic that Considers Transsexuality as a Pathology and the Lack of Effective Health Services and Professional Practices in the Attention to the Trans Population. We concluded that rejecting the plural experiences of trans people in the access to health services and pathways hinders the qualified assistance from professionals and institutions, be they public or private, putting this population at risk and making preventive actions targeted at integral healthcare more difficult to achieve. We argue that it is necessary to create practices in health in both the public sphere and society as a whole, guaranteeing rights and reducing prejudice.

Este artículo buscó realizar una revisión integrativa sobre las dificultades enfrentadas por la población trans en el acceso a servicios de salud. Se seleccionó 15 artículos publicados de 2012 a 2021 en las basis de datos Índice Bibliográfico Español en Ciencias de la Salud (IBECS), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) y Scientific Electronic Library Online (SciELO). Para discutir los resultados, se estableció cuatro categorías de análisis: Panorama de los Principales Marcos Legislativos sobre los Derechos de la Población Trans; Estigmatización y Prejuicio contra la Población Trans; La Lógica Binaria y Biológica como Promotora de la Patologización y Medicalización de la Transexualidad y Carencia de Efectividad en los Servicios y Prácticas Profesionales de Salud en la Atención a la Población Trans. Se concluyo que el rechazo de la vivencia plural de la población trans en el acceso a servicios y rutas de salud genera prejuicios a la asistencia cualificada de profesionales e instituciones, sean ellas públicas o privadas, creando situaciones de riesgo y dificultando acciones preventivas direccionadas a la manutención y cuidado integral de salud. Es necesario construir prácticas en salud entre el poder público y la sociedad, garantizando derechos y reduciendo prejuicios.O acesso aos serviços de saúde constitui um direito constitucional de todas as pessoas. O presente artigo objetivou realizar uma revisão integrativa acerca das dificuldades enfrentadas pela população trans no acesso aos serviços de saúde. Foram consultadas as bases de dados Índice Bibliográfico Español en Ciencias de la Salud (IBECS), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e Scientific Electronic Library Online (SciELO), o que permitiu selecionar 15 artigos completos publicados entre 2012-2021. Para a discussão dos resultados, foram estabelecidas quatro categorias de análise: Panorama dos Principais Marcos Legislativos sobre os Direitos da População Trans; Estigmatização e Discriminação da População Trans; A Lógica Binarista e Biologizante como Promotora da Patologização e Medicalização da Transexualidade e Carência de Efetividade dos Serviços em Saúde e das Práticas Profissionais no Atendimento à População Trans. Concluiu-se que a rejeição das vivências plurais da população trans no acesso aos serviços e itinerários de saúde geram prejuízos quanto à assistência qualificada de profissionais e instituições, sejam públicas ou privadas, colocando-as em situação de risco e dificultando ações preventivas voltados à manutenção e cuidado integral da saúde.
