Prevalence of delirium in the adult intensive care unit of a public hospital in Minas Gerais

Resumo: | Introduction: Delirium is a severe neuropsychiatric syndrome. It is characterized by fluctuating alterations in the level of consciousness, with an acute onset of symptoms. Objective: to identify the prevalence of delirium in (ICU) adults and make the data available to the institution, with the aim of providing information to add to the care and clinical management of these patients. Method: This is a descriptive observational study, with a quantitative approach, carried out in an adult ICU, in a hospital in the interior of Minas Gerais. Data collection was carried out between June and September 2019, through the application of the Confusion Assessment Method in an Intensive Care Unit (CAM-ICU) scale. Results: The sample consisted of 97 patients who met the inclusion criteria of the study, of which 47.42% had a positive diagnosis for delirium in at least one evaluation using the CAM - ICU scale. There was no significant difference in prevalence regarding gender. There was a greater predominance of cases in individuals aged between 60 and 79 years, with the mean age of the study population being 66.3 years. The causes of hospitalization of the selected patients were predominantly cardiovascular, neurological and traumatic problems. About 86.95% of patients with a positive assessment for delirium used mechanical ventilation (MV) and sedation at some point during hospitalization. Conclusion: The prevalence of delirium in the studied ICU is high, reinforcing the need to create protocols in order to identify it early and treat it appropriately. |

Se caracteriza por cambios fluctuantes en el nivel de conciencia, con un inicio agudo de los síntomas. Objetivo: identificar la prevalencia de delirio en adultos (UCI) y poner los datos a disposición de la institución, con el objetivo de brindar información para sumar a la atención y manejo clínico de estos pacientes. Método: Se trata de un estudio observacional descriptivo, con abordaje cuantitativo, realizado en una UTI de adultos, en un hospital del interior de Minas Gerais. La recolección de datos se realizó entre junio y septiembre de 2019, mediante la aplicación de la escala Confusion Assessment Method in an Intensive Care Unit (CAM-ICU). Resultados: La muestra estuvo conformada por 97 pacientes que cumplieron con los criterios de inclusión del estudio, de los cuales el 47,42% tuvo diagnóstico positivo para delirio en al menos una evaluación mediante la escala CAM - UCI. No hubo diferencia significativa en la prevalencia según el sexo. Hubo mayor predominio de casos en individuos de 60 a 79 años, con una edad media de la población estudiada de 66,3 años. Las causas de hospitalización de los pacientes seleccionados fueron predominantemente problemas cardiovasculares, neurológicos y traumáticos. Aproximadamente el 86,95% de los pacientes con valoración positiva de delirio utilizaron ventilación mecánica (VM) y sedación en algún momento de su hospitalización. Conclusión: La prevalencia de delirio en la UTI estudiada es alta, lo que refuerza la necesidad de crear protocolos para identificarlo precozmente y tratarlo adecuadamente.Introdução: Delirium consiste em uma síndrome neuropsiquiátrica grave. Caracteriza-se pela flutuação de alterações no nível de consciência, de início agudo dos sintomas. Objetivo: Identificar a prevalência de delirium na (UTI) adultos e disponibilizar os dados à instituição, com o intuito de fornecer informações para agregar à assistência e manejo clínico desses pacientes. Método: Trata-se de um estudo do tipo observacional descritivo, com abordagem quantitativa, realizado em uma UTI adulto de um hospital do interior de Minas Gerais. A coleta de dados foi realizada entre junho e setembro de 2019, através da aplicação da escala Confusion Assessment Method in a Intensive Care Unit (CAM-ICU). Resultados: A amostra foi composta por 97 pacientes, que atendiam aos critérios de inclusão do estudo, dos quais 47,42% (46 indivíduos), apresentaram diagnóstico positivo para delirium em ao menos uma avaliação através da escala CAM – ICU. Não houve diferença significativa da prevalência quanto ao gênero. Ocorreu maior predomínio de casos nos indivíduos com idade entre 60 e 79 anos, sendo a idade média da população do estudo de 66,3 anos. As causas de internação dos pacientes selecionados, foram predominantemente problemas cardiovasculares, neurológicos e traumáticos. Cerca de 86,95 % (n=40) dos pacientes com avaliação positiva para delirium utilizou ventilação mecânica (VM) e sedação em algum momento da internação.
