Epidemiological and spatial profile of acquired syphilis: a sectional study based on a historical series

Resumo: Despite being preventable and curable, syphilis is a serious public health problem worldwide. This study seeks to describe the epidemiological and spatial profile of syphilis acquired in the North of Minas Gerais in the years 2016 to 2020. It is a cross-sectional, retrospective and descriptive study. Data collection was carried out using secondary data extracted from the Notification Diseases Information System (SINAN). A descriptive analysis of the variables was performed to characterize the epidemiological profile by checking frequency and percentage. Fisher's exact statistical test was applied to verify the association of variables, assuming a significance of 95% (p<0.05). The prevalence rate of the historical series studied and the detection rates for each year were calculated and presented in a geospatial illustration for analysis of the geographical and temporal distribution of cases. 1613 notification forms were analyzed, noting a majority male, brown, with complete high school, residing in the urban area, young people between 20 and 29 years old and with evolution towards cure. There were associations between the sex variable and the age group and race variables. The detection rate showed an upward trend between 2016 and 2018, showing a higher concentration of cases in the regional hub. The need to focus on educational health actions is revealed, with the objective of promoting sexual health and offering effective measures to combat syphilis in the most affected population and professional education to qualify the information. It hopes that this study will contribute to propositional strategies and the promotion of similar studies.

Este estudio busca describir el perfil epidemiológico y espacial de la sífilis adquirida en el Norte de Minas Gerais en los años 2016 a 2020. Es un estudio transversal, retrospectivo y descriptivo. La recolección de datos se realizó a partir de datos secundarios extraídos del Sistema de Información de Enfermedades de Notificación (SINAN). Se realizó un análisis descriptivo de las variables para caracterizar el perfil epidemiológico comprobando frecuencia y porcentaje. Se aplicó la prueba estadística exacta de Fisher para verificar la asociación de variables, asumiendo una significación del 95% (p<0,05). La tasa de prevalencia de la serie histórica estudiada y las tasas de detección de cada año se calcularon y presentaron en una ilustración geoespacial para el análisis de la distribución geográfica y temporal de los casos. Se analizaron 1613 formularios de notificación, observándose una mayoría de sexo masculino, morenos, con secundaria completa, residente en el casco urbano, jóvenes entre 20 y 29 años y con evolución hacia la cura. Hubo asociaciones entre la variable sexo y las variables grupo de edad y raza. La tasa de detección mostró una tendencia ascendente entre 2016 y 2018, mostrando una mayor concentración de casos en el eje regional. Se revela la necesidad de focalizar acciones educativas en salud, con el objetivo de promover la salud sexual y ofrecer medidas efectivas de combate a la sífilis en la población más afectada y formación profesional para cualificar la información. Realizada análise descritiva das variáveis para caracterização do perfil epidemiológico por verificação de frequência e percentual. Foi aplicado teste estatístico de Exato de Fisher para verificação de associação das variáveis, assumindo significância de 95% (p<0,05). Foram calculadas a taxa de prevalência da série histórica estudada e as taxas de detecção para cada ano e apresentadas em ilustração geoespacial para análise da distribuição geográfica e temporal dos casos. Foram analisadas 1613 fichas de notificação constatando prevalência do sexo masculino, pardos, com ensino médio completo, residente na zona urbana, jovens de 20 a 29 anos e com evolução para a cura. Verificou-se associações entre a variável sexo com as variáveis faixa etária e raça.  A taxa de detecção apresentou evolução ascendente entre os anos de 2016 a 2018, mostrando maior concentração dos casos no polo regional. Revela-se a necessidade de priorizar ações educativas em saúde, com o objetivo de promover a saúde sexual e oferecer medidas efetivas no combate à sífilis na população mais acometida e educação profissional para qualidade da informação.
