Importance of immunohematological techniques in the blood transfusion process

Resumo: | Transfusion therapy results in the transfusion of specific blood components, which the patient needs to replace, opposing the use of whole blood. This procedure aims to benefit several patients, and to optimize the stocks of blood banks. The presence of anti-erythrocyte antibodies in the serum of patients can cause hemolytic reactions. The identification of these antibodies is extremely necessary for the search for compatible blood components and, consequently, the prevention of transfusion reactions. The objective of this work is to describe the importance of pre-transfusion immunohematological techniques in the blood transfusion process. This is a qualitative bibliographic review based on scientific production based on studies already published. The decision to transfuse a patient must consider the risks and benefits and assess whether there is a real need for blood transfusion. The evaluation occurs through hemoglobin values (for red blood cell transfusions) and platelet values (for blood platelet transfusions) and the clinical aspects of the patient. The introduction of laboratory tests and better donor screening have significantly reduced mortality rates and risks of infections related to this procedure and the most frequent complications today are related to non-infectious causes. Patients who frequently perform blood transfusions may become sensitized, producing alloantibodies, due to the presence of anti-erythrocyte antigens present in the transfused blood bags. These patients have a greater risk of developing transfusion reactions, therefore, the importance of pre-transfusion immunohematological techniques whose main function is to track the presence of irregular antibodies in the recipients and find compatible bags for blood transfusion, decreasing the risk of transfusion reactions and increasing patient safety. |

Este procedimiento tiene como objetivo beneficiar a varios pacientes y optimizar las reservas de bancos de sangre. La presencia de anticuerpos anti-eritrocitos en el suero de los pacientes puede causar reacciones hemolíticas. La identificación de estos anticuerpos es extremadamente necesaria para buscar componentes sanguíneos compatibles y, en consecuencia, para evitar reacciones a la transfusión. El objetivo de este trabajo es describir la importancia de las técnicas inmunohematológicas previas a la transfusión en el proceso de transfusión de sangre. Esta es una revisión bibliográfica cualitativa basada en la producción científica basada en estudios ya publicados. La decisión de transfundir a un paciente debe considerar los riesgos y beneficios y evaluar si existe una necesidad real de transfusión de sangre. La evaluación se realiza a través de los valores de hemoglobina (para transfusiones de glóbulos rojos) y valores de plaquetas (para transfusiones de concentrados de plaquetas) y los aspectos clínicos del paciente. La introducción de pruebas de laboratorio y una mejor detección de donantes han reducido significativamente las tasas de mortalidad y los riesgos de infecciones relacionadas con este procedimiento y las complicaciones más frecuentes en la actualidad están relacionadas con causas no infecciosas. Los pacientes que realizan transfusiones de sangre con frecuencia pueden sensibilizarse y producir aloanticuerpos, debido a la presencia de antígenos anti-eritrocitos presentes en las bolsas de sangre transfundidas. Estos pacientes tienen un mayor riesgo de desarrollar reacciones a la transfusión, por lo tanto, la importancia de las técnicas inmunohematológicas previas a la transfusión cuya función principal es rastrear la presencia de anticuerpos irregulares en los receptores y encontrar bolsas compatibles para la transfusión de sangre, disminuyendo el riesgo de reacciones a la transfusión y aumentando la seguridad del paciente.A Terapia Transfusional resulta na transfusão de componentes específicos do sangue, dos quais o paciente necessita repor, opondo-se a utilização de sangue total. Este procedimento visa o beneficio de vários pacientes, e a otimização dos estoques dos bancos de sangue. A presença de anticorpos antieritrocitários no soro de pacientes podem ocasionar reações hemolíticas. É extremamente necessária a identificação desses anticorpos para a busca de hemocomponentes compatíveis e consequentemente a prevenção de reações transfusionais. O objetivo deste trabalho é de descrever a importância das técnicas imunohematológicas pré transfusionais no processo de transfusão de hemocomponentes. Trata-se de uma revisão bibliográfica de caráter qualitativo que se baseia na produção científica a partir de estudos já publicados. A decisão de transfundir um paciente deve considerar os riscos e benefícios e avaliar se há realmente necessidade de uma hemotransfusão. A avaliação ocorre através dos valores de hemoglobina (para transfusões de concentrado de hemácias) e valores de plaquetas (para transfusões de concentrado de plaquetas) e os aspectos clínicos do paciente. A introdução de testes laboratoriais e uma melhor triagem dos doadores reduziram significativamente as taxas de mortalidade e riscos de infecções relacionadas a este procedimento e as complicações mais frequentes atualmente são relacionadas a causas não infecciosas. Pacientes que realizam transfusões sanguíneas com frequência podem sensibilizar-se, produzindo aloanticorpos, devido à presença de antígenos antieritrocitários presentes nas bolsas de sangue transfundidas. Esses pacientes apresentam um risco maior no desenvolvimento de reações transfusionais, por isso, a importância das técnicas imunohematológicas pré transfusionais que tem como função principal rastrear a presença de anticorpos irregulares nos receptores e encontrar bolsas compatíveis para a hemotransfusão, diminuindo o risco de reações transfusionais e aumentando a segurança do paciente.
