Notification of transfusion reactions in a tertiary hospital

Resumo: Blood transfusion is an effective therapeutic method used in cases of large-volume hemorrhages, cancer treatments, coagulopathies, among other causes.  However, it can cause risks such as allergic reactions, febrile and circulatory volume overloads. The aim of this research is to analyze the occurrences of transfusion reactions in patients receiving blood components, identifying the frequency of the main immediate reactions, as well as severity and associated factors.  This is a retrospective and descriptive study about blood transfusions and transfusion reactions that occurred in patients admitted to the Southwestern Regional Hospital Dr Walter Alberto Pecóits (HRSWAP) from 2018 to 2020. The variables collected included age, gender, unit of occurrence, diagnostic hypothesis at admission, signs and symptoms observed, time of onset of manifestations, severity and type of hemocomponent.  In total, 1,837 patients were blood transfusion recipients from 2018 to 2020. A total of 16 immediate transfusion reaction forms were identified, in which the most frequent clinical manifestations were: body temperature elevation in at least 1ºC (75%), shorthand (25%), hypertension (18.75%) and hives (18.75%); 93.75% of the reactions classified as mild. Moreover, most of the reactions occurred in transfusions to patients who were in the Surgical Clinic and Urgency and Emergency sectors, reference areas for the service studied. In addition to verifying the indications and transfusion reactions, the study presented limitations related to the inadequate filling out of the notification forms. Thus, the work becomes relevant by attracting visibility to good hemotherapeutic conducts, considering the specificities of the hospital addressed.

Sin embargo, puede causar riesgos como reacciones alérgicas, sobrecargas de volumen febril y circulatorio. El objetivo de esta investigación es analizar la ocurrencia de reacciones transfusionales en pacientes que reciben componentes sanguíneos, identificando la frecuencia de las principales reacciones inmediatas, así como la gravedad y los factores asociados. Se trata de un estudio retrospectivo y descriptivo sobre transfusiones de sangre y reacciones transfusionales que se produjeron en pacientes ingresados en el Hospital Regional del Suroeste Dr. Walter Alberto Pecóits (HRSWAP) de 2018 a 2020. Las variables recogidas incluyeron edad, sexo, unidad de ocurrencia, hipótesis diagnóstica al ingreso, signos y síntomas observados, momento de inicio de las manifestaciones, gravedad y tipo de hemocomponente. En total, 1.837 pacientes fueron receptores de transfusiones de sangre de 2018 a 2020. Se identificaron un total de 16 formas de reacción inmediata a la transfusión, en las que las manifestaciones clínicas más frecuentes fueron: elevación de la temperatura corporal en al menos 1ºC (75%), taquigrafía (25%), hipertensión (18,75%) y urticaria (18,75%); El 93,75% de las reacciones se clasificaron como leves. Además, la mayoría de las reacciones se produjeron en transfusiones a pacientes que se encontraban en los sectores de Clínica Quirúrgica y Urgencias y Urgencias, áreas de referencia para el servicio estudiado. Además de verificar las indicaciones y las reacciones a la transfusión, el estudio presentó limitaciones relacionadas con el llenado inadecuado de los formularios de notificación. Así, el trabajo cobra relevancia al dar visibilidad a las buenas conductas hemoterapéuticas, teniendo en cuenta las especificidades del hospital al que se dirige.A transfusão sanguínea é um método terapêutico eficaz utilizado em casos de hemorragias de grande volume, tratamentos oncológicos, coagulopatias, entre outras causas. O objetivo da pesquisa é analisar as ocorrências de reações transfusionais em pacientes receptores de hemocomponentes, identificando a frequência das principais reações imediatas, bem como gravidade e fatores associados. Trata-se de um estudo retrospectivo e descritivo acerca das hemotransfusões e reações transfusionais que aconteceram em pacientes internados no no Hospital Regional do Sudoeste Dr Walter Alberto Pecóits (HRSWAP) no período de 2018 a 2020. As variáveis coletadas incluíram idade, sexo, unidade de ocorrência, hipótese diagnóstica na admissão, sinais e sintomas observados, tempo de aparecimento das manifestações,  gravidade e tipo de hemocomponente. Ao total, 1837 pacientes foram receptores de transfusão sanguínea de 2018 a 2020. Foram identificadas 16 fichas de reação transfusional imediata, em que as manifestações clínicas mais frequentes foram: elevação de temperatura corporal em pelo menos 1ºC (75%), taquicardia (25%), hipertensão (18.75%) e urticária (18.75%); com 93,75% das reações classificadas como leves. Ademais, a maior parte das reações ocorreu em transfusões a pacientes que estavam nos setores Clínica Cirúrgica e Urgência e Emergência, áreas de referência para o serviço estudado. Além de verificar as indicações e reações transfusionais, o estudo apresentou limitações relacionadas ao preenchimento inadequado das fichas de notificação.
