Resumo: Objectives: to verify the fulfillment of the fourth and fifth steps of the Baby-Friendly Hospital Initiative in a reference maternity hospital in northeastern Brazil. Methodology: Descriptive research of quantitative nature, conducted between March and July 2019, with 380 puerperal women interviewed in the hospital wards to fill out a form. The data were tabulated according to absolute and relative frequencies. Results: It was found that 17% of the babies were placed to breastfeed in the delivery room in fulfillment of the fourth step of the Baby-Friendly Hospital Initiative, 82% of the mothers received help to breastfeed in fulfillment of the fifth step; 79% of mothers were between 20 and 30 years old, 61% were mixed, 71% were married, 57% attended high school, 69% were housewives, 48% with income between 1 and 2 minimum wages, 42% had 2 children, 54% did not have their own residence, all had prenatal care, 66% had more than 6 consultations, 79% was oriented about breastfeeding. Conclusion: The indexes found for the fourth and fifth steps of the Baby-Friendly Hospital Initiative are below those recommended by the Ministry of Health. These results reinforce the importance of effective support and promotion of breastfeeding as the focus of the nursing team.

Metodología: Investigación descriptiva de carácter cuantitativo, realizada entre marzo y julio de 2019, con 380 mujeres puerperales entrevistadas en las salas hospitalarias para llenar un formulario. Los datos se tabularon según frecuencias absolutas y relativas. Resultados: Se encontró que el 17% de los bebés fueron colocados para amamantar en la sala de partos en cumplimiento del cuarto paso de la Iniciativa Hospitalaria Amiga del Bebé, el 82% de las madres recibieron ayuda para amamantar en cumplimiento del quinto paso; El 79% de las madres tenían entre 20 y 30 años, El 61% eran mixtos, el 71% estaban casados, el 57% asistían a la escuela secundaria, el 69% eran amas de casa, el 48% con ingresos entre 1 y 2 salarios mínimos, el 42% tenían a 2 hijos, el 54% no tenían su propia residencia, todos tenían atención prenatal, el 66% tenían más de 6 consultas, el 79% estaba orientado a la lactancia materna. Conclusión: Los índices encontrados para los pasos cuarto y quinto de la Iniciativa Hospitalaria Amiga del Bebé están por debajo de los recomendados por el Ministerio de Salud. Estos resultados refuerzan la importancia del apoyo efectivo y la promoción de la lactancia materna como foco del equipo de enfermería.Objetivos: Verificar o cumprimento do quarto e quinto passos da Iniciativa Hospital Amigo da Criança em uma maternidade de referência no nordeste brasileiro. Metodologia: Pesquisa descritiva de natureza quantitativa, realizada entre março e julho de 2019, com 380 puérperas entrevistadas nas enfermarias do hospital para preenchimento de um formulário. Os dados foram tabulados de acordo com as frequências absolutas e relativas. Resultados: Encontrou-se que 17% dos bebês foram colocados para mamar na sala de parto em cumprimento do quarto passo da Iniciativa Hospital Amigo da Criança, 82% das mães receberam auxílio para amamentar em cumprimento do quinto passo; 79% das mães tinham entre 20 a 30 anos, 61% eram pardas, 71% eram casadas, 57% cursaram Ensino Médio, 69% eram donas de casa, 48% com renda entre 1 e 2 salários mínimos, 42% tinha 2 filhos, 54% não tinham residência própria, todas fizeram pré-natal, 66% fizeram mais de 6 consultas, 79% foi orientada sobre amamentação. Conclusão: Os índices encontrados referentes ao quarto e quintos passos da Iniciativa Hospital Amigo da Criança estão abaixo dos recomendados pelo Ministério da Saúde.
