Perception of patients undergoing peritoneal dialysis regarding the risks of infections during home aseptic practices

Resumo: Peritoneal Dialysis is a treatment modality for patients with Chronic Kidney Disease and presents benefits such as the return of patients to a practically normal life due to the autonomy of self-care and easy of treatment at home. The aim of the study was to understand the perception of patients undergoing peritoneal dialysis treatment regarding the risks of infections during home aseptic practices. It´s descriptive study of a qualitative approach developed conducted from March to June 2019 in a Private Clinic of Nephrology of Teresina-PI. The study included Adult patients on Peritoneal Dialysis treatment older than 18 years were included in the study and excluded those who were debilitated, with cognitive or psychological restrictions. For data collection it was used a semi-structured interview script with data on the sociodemographic characteristics of the interviewed group, and the second with open questions about the risks of infection and care with the PD catheter. Results were interpreted and discussed according to Content Analysis. Most patients were found to have infection at the catheter outlet and in some cases to peritonitis, which were possibly acquired by procedural errors during self-care with the Peritoneal Dialysis catheter. Even after receiving instructional training by the health team about the treatment, material used, home care, aseptic methods, among others, the patients had difficulties in the handling of the DP catheter. It is necessary that the multidisciplinary health team rethink the logic of knowledge construction of patients with peritoneal dialysis, seeking to identify the approach that guides treatment when performed at home.

El objetivo del estudio fue conocer la percepción de los pacientes sometidos a diálisis peritoneal sobre los riesgos de infecciones durante las prácticas asépticas domiciliarias. Estudio exploratorio con abordaje descriptivo cualitativo, realizado de marzo a junio de 2019 en una Clínica Privada de Nefrología en Teresina-PI. Se incluyeron en el estudio pacientes adultos sometidos a tratamiento de diálisis peritoneal mayores de 18 años, excluyendo a aquellos que estaban deteriorados, con restricciones cognitivas o psicológicas. Para la recogida de datos se utilizó un guión de entrevista semiestructurado con datos de las características sociodemográficas del grupo entrevistado y preguntas abiertas sobre los riesgos de infección y los cuidados con el catéter de DP. Los resultados fueron interpretados y discutidos de acuerdo con el Análisis de Contenido. Se encontró que la mayoría de los pacientes presentaban infección en el orificio de salida del catéter y en algunos casos peritonitis, posiblemente adquiridas por errores de procedimiento durante el autocuidado con el catéter de Diálisis Peritoneal. A pesar de recibir capacitación instruccional por parte del equipo de salud sobre el tratamiento, material utilizado, cuidados domiciliarios, métodos asépticos, entre otros, los pacientes tuvieron dificultades en el manejo del catéter DP. Es necesario que el equipo multidisciplinario de salud repense la lógica de construir el conocimiento de los pacientes con Diálisis Peritoneal, buscando identificar el abordaje que guía el tratamiento cuando se realiza en el hogar.A Diálise Peritoneal é uma modalidade de tratamento para os pacientes com Doença Renal Crônica e apresenta benefícios como o retorno dos pacientes a uma vida praticamente normal em função da autonomia do autocuidado e da facilidade de tratamento em domicílio. O objetivo do estudo foi conhecer a percepção dos pacientes em tratamento de diálise peritoneal quanto aos riscos de infecções durante as práticas assépticas domiciliares. Estudo exploratório com abordagem qualitativa descritiva, realizado nos meses de março a junho de 2019 em uma Clínica Particular de Nefrologia de Teresina-PI. Foram incluídos no estudo pacientes adultos em tratamento de Diálise Peritoneal com idade acima de 18 anos e, excluídos aqueles que se encontravam debilitados, com restrições cognitivas ou psicológicas. Para coleta dos dados foi utilizado um roteiro de entrevista semiestruturado com dados sobre as características sociodemográficas do grupo entrevistado e questões abertas sobre os riscos de infecção e os cuidados com o cateter de DP. Os resultados foram interpretados e discutidos de acordo com a Análise de Conteúdo. Verificou-se que a maioria dos pacientes apresentou infecção no orifício de saída do cateter e em alguns casos a peritonite, que possivelmente foram adquiridas por erros de procedimento durante o autocuidado com o cateter de Diálise Peritoneal. Mesmo com recebimento de um treinamento instrutivo pela equipe de saúde sobre o tratamento, material utilizado, cuidados domiciliares, métodos assépticos, entre outros, os pacientes apresentaram dificuldades no manuseio do cateter DP.
