Prevalence analysis, laboratory diagnosis and treatment of syphilis in pregnancy: a literature review

Resumo: The research aimed to analyze the prevalence, laboratory diagnosis and treatment of syphilis during pregnancy. It was an integrative literature review, in which the SciELO, LILACS and VHL databases were used, by crossing the descriptors: Laboratory diagnosis, Pregnancy, Prevalence, Syphilis and Treatment. The research was guided by the following questions: How is the treatment, laboratory diagnosis and the prevalence of syphilis in pregnancy currently? What are the public health policies of the Unified Health System (SUS) aimed at gestational syphilis. A total of 5,364 publications were found, which, after meeting the inclusion and exclusion criteria, were filtered and 10 publications were selected, which made up the final sample of the study. The articles chosen were between the years 2016 to 2022. The main findings of the study showed that Gestational Syphilis and Congenital Syphilis have victimized mothers and babies and challenged the health system with regard to tracking, treatment and eradication of the disease . It is evident the need for actions aimed at reducing vertical transmission of syphilis, expanding access to diagnostic methodologies, in addition to serological screening, these attributions reinforce the prevention of the disease and the reduction of possible sequelae. It is concluded, therefore, that poor quality and inadequate prenatal care contributes to the emergence of failures in the treatment of pregnant women, which may possibly result in congenital cases of syphilis. Therefore, it is of fundamental importance to promote health actions that aim to eliminate the vertical transmission of syphilis.

Fue una revisión integrativa de la literatura, en la que se utilizaron las bases de datos SciELO, LILACS y BVS, mediante el cruce de los descriptores: Diagnóstico de laboratorio, Embarazo, Prevalencia, Sífilis y Tratamiento. La investigación estuvo guiada por las siguientes preguntas: ¿Cómo es el tratamiento, el diagnóstico de laboratorio y la prevalencia de la sífilis en el embarazo actualmente? Cuáles son las políticas de salud pública del Sistema Único de Salud (SUS) dirigidas a la sífilis gestacional. Se encontraron un total de 5.364 publicaciones que, tras cumplir los criterios de inclusión y exclusión, se filtraron y se seleccionaron 10 publicaciones, que conformaron la muestra final del estudio. Los artículos elegidos fueron entre los años 2016 a 2022. Los principales hallazgos del estudio mostraron que la Sífilis Gestacional y la Sífilis Congénita han victimizado a madres y bebés y desafiado el sistema de salud en lo que respecta al seguimiento, tratamiento y erradicación de la enfermedad . Es evidente la necesidad de acciones encaminadas a la reducción de la transmisión vertical de la sífilis, ampliando el acceso a metodologías de diagnóstico, además del tamizaje serológico, estas atribuciones refuerzan la prevención de la enfermedad y la reducción de posibles secuelas. Se concluye, por lo tanto, que la mala calidad y el control prenatal inadecuado contribuyen a la aparición de fallas en el tratamiento de las gestantes, lo que posiblemente puede resultar en casos congénitos de sífilis. Por lo tanto, es de fundamental importancia promover acciones de salud que apunten a eliminar la transmisión vertical de la sífilis.A pesquisa objetivou analisar a prevalência, diagnóstico laboratorial e o tratamento da sífilis na gestação. Tratou-se de uma revisão integrativa da literatura, no qual foram utilizadas as bases de dados SciELO, LILACS e o BVS, mediante cruzamento dos descritores: Diagnóstico laboratorial, Gestação, Prevalência, Sífilis e Tratamento. A pesquisa foi norteada pelos seguintes questionamentos: Como ocorre o tratamento, diagnóstico laboratorial e a prevalência da sífilis na gestação atualmente? Quais são as políticas públicas de saúde do Sistema Único de Saúde (SUS) direcionadas a sífilis gestacional. Foram encontradas 5.364 publicações, que após passarem pelos critérios de inclusão e exclusão, foram filtradas e selecionadas 10 publicações, essas compuseram a amostra final do estudo. Os artigos escolhidos estavam entre os anos de 2016 a 2022. Os principais achados do estudo, mostraram que a Sífilis Gestacional e a Sífilis Congênita tem vitimado mães e bebês e desafiado o sistema de saúde no que se refere ao rastreamento, tratamento e erradicação da doença. É evidente a necessidade de ações que objetivem a redução da transmissão vertical da sífilis, ampliação do acesso as metodologias diagnósticas, além de rastreamento sorológico, essas atribuições reforçam a prevenção da doença e a redução de possíveis sequelas. Conclui-se, portanto, que um pré-natal sem qualidade e inadequado contribui para o surgimento de falhas no tratamento de mulheres grávidas, o que possivelmente pode resultar em casos congênitos da Sífilis.
