The importance of multidisciplinarity in surgical and supportive conduct in severe odontogenic infection: Case Report

Resumo: | Odontogenic infections can evolve quickly and offer a high risk of death, since they are commonly associated with extensive tissue deterioration, in addition to their location, in close contact with important anatomical structures such as the airways, which can lead to their obstruction. The objective is to report, possible difficulties in the management of a patient, due to the Angina de Ludwig and consequent deviation of the airways, and the importance of the multidisciplinary medical-dental approach. Male patient, 33 years old, complaining of pain in the region of the lower right third molar, with evolution of one week, with extraoral edema that progressed gradually, was submitted to drainage still in the emergency, then the team proceeded with his hospitalization, the next day, the patient was taken to the operating room for further drainage, and extraction of dental elements associated with infection. At the time of intubation, there was difficulty due to the displacement of the tongue, which prevented adequate passage of the tube, directing the treatment to a tracheostomy. It is essential that the cause be removed, in order for it to regress and in view of the surgical need, it is essential that in the preoperative evaluation, the medical team evaluates possible airway compromises that may culminate in difficulties for intubation. There is no consensus about the approach for intubation in angina conditions due to a wide variety of situations, however, there is a need for a multidisciplinary medical-dental approach in order to achieve a fast and accurate approach. |

Así, el objetivo es informar, las posibles dificultades en el manejo de un paciente, debido a la Angina de Ludwig y la consiguiente desviación de la vía aérea, así como la importancia del abordaje médico-odontológico multidisciplinario. Paciente de sexo masculino, 33 años, que consulta por dolor en la región del tercer molar inferior derecho, con evolución de una semana, con edema extraoral que progresa gradualmente, fue sometido a drenaje aún en urgencias, luego el equipo procedió a su internación. , al día siguiente, la paciente fue trasladada al quirófano para su posterior drenaje y extracción de los elementos dentales asociados a la infección. En el momento de la intubación, hubo dificultad debido al desplazamiento de la lengua, lo que impidió el paso del tubo, dirigiendo el tratamiento a una traqueotomía. Es fundamental que se elimine la causa, para que ésta retroceda y com necesidad quirúrgica es fundamental que en la evaluación preoperatoria el equipo médico evalúe los posibles compromisos de la vía aérea que pueden culminar en dificultades para la intubación. No existe consenso sobre el mejor abordaje para la intubación en condiciones de angina debido a una amplia variedad de situaciones clínicas, sin embargo, existe la necesidad de un abordaje médico-dental multidisciplinario para lograr un abordaje rápido y preciso.As infecções odontogênicas podem evoluir rapidamente e oferecer grande risco de morte, visto que comumente são associadas à extensas deteriorações dos tecidos, além da sua localização, em íntimo contato com estruturas anatômicas importantes como vias aéreas, podendo levar a sua obstrução. Dessa forma, objetiva-se relatar, possíveis dificuldades do manejo de um paciente, devido ao quadro de Angina de Ludwig e consequente desvio de vias aéreas, e também a importância da abordagem multidisciplinar médico-odontológica. Paciente do sexo masculino, 33 anos de idade, queixando-se de dor em região de terceiro molar inferior direito, com evolução de uma semana, com edema extraoral que progredia gradativamente, foi submetido à drenagem ainda no pronto-atendimento, em seguida a equipe procedeu com sua internação, no dia seguinte, o paciente foi levado ao centro cirúrgico para realização de nova drenagem, e exodontia de elementos dentários associados a infecção. No momento de intubação houve dificuldade para mesma devido ao deslocamento da língua, que impossibilitava a adequada passagem do tubo, direcionando o tratamento para uma traqueostomia. É imprescindível que seja realizada a remoção da causa, para que haja a sua regressão e frente a necessidade cirúrgica, é fundamental que na avaliação pré-operatória, a equipe médica avalie os possíveis comprometimentos de via aérea que possam culminar em dificuldades para a intubação.
