Perception of families of patients with COVID-19 about hospitalization in adult intensive care

Resumo: Objective: To know the perception of family members of COVID-19 hospitalized patients about the period of hospitalization in an Adult Intensive Care Unit. Methodology: Descriptive and qualitative study with adult family members of patients with COVID-19 admitted to an Adult Intensive Care Unit of a tertiary public hospital in Porto Alegre (RS), Brazil. A sheet with sociodemographic data and a semi-structured questionnaire were used. The interviews were conducted between November and December/2020 by phone call and the sample was determined by data saturation. Bardin’s content analysis was used. Results: Nine family members aged between 23 and 56 years old participated. Predominated women, mostly daughters. The number of days in isolation ranged from 8 to 31. Five categories emerged: The Intensive Care Unit as a death sentence; The impact of a new disease; Family distance and exclusion; Support networks; The imminence of death. Conclusion: Hospitalizations due to COVID-19 in Intensive Care Unit were permeated by fear and anguish of family members, increased because it is a new and incurable disease. The need for isolation made physical contact impossible, impacting the understanding of the condition and the elaboration of mourning. The support of team healthcare, other family members and friends, faith and religiosity emerged as protective mechanisms. Anticipatory grief, fear of death without saying goodbye and multiple losses are central to grief. Emotional support for family members is essential, in order to promote psychological well-being and prevent psychological injuries and complicated grief.

Fue utilizada una ficha de datos sociodemográficos y un cuestionario semiestructurado. Las entrevistas fueron realizadas entre noviembre y deciembre/2020 por llamada telefonica y la muestra fue determinada por saturación. Fue utilizado el análisis de contenido de Bardin. Resultados: Participaron nueve familiares entre 23 y 56 años. Han predominado las mujeres, la mayoría hijas. El número de días de aislamiento varió de 8 a 31. Emergieron cinco categorías: Unidad de Cuidados Intensivos como sentencia de muerte; El impacto de una nueva enfermedad; Distancia y exclusión familiar; Redes de apoyo; La inminencia de la muerte. Conclusión: Las hospitalizaciones en la Unidad de Cuidados Intensivos por COVID-19 fueron permeadas por miedo y angustia de los familiares, incrementados por tratarse de una enfermedad nueva e incurable. La necesidad de aislamiento imposibilitó el contacto físico, impactando la comprensión de la condición y la elaboración de luto. El apoyo del equipo, de otros familiares y amigos, la fe y la religiosidad aparecieron como mecanismos de protección. El luto anticipatorio, el miedo de la muerte sin despedida y las pérdidas múltiples son centrales en el luto. El apoyo emocional a los familiares se considera fundamental para promover el bienestar psicológico y prevenir lesiones psicológicas y lutos complicados.Objetivo: Conhecer a percepção de familiares de pacientes internados por COVID-19 em Unidade de Terapia Intensiva Adulto sobre o período de internação. Emergiram cinco categorias: A Unidade de Terapia Intensiva como sentença de morte; O impacto de uma doença nova; Distância e exclusão da família; Redes de apoio; A iminência da morte. Conclusão: As internações em Unidade de Terapia Intensiva por COVID-19 foram permeadas por medo e angústia dos familiares, incrementados por tratar-se de uma doença nova e sem cura. A necessidade de isolamento impossibilitou o contato físico, impactando na compreensão do quadro e na elaboração do luto. O apoio da equipe, de outros familiares e amigos, fé e religiosidade apareceram como mecanismos protetivos. Luto antecipatório, medo da morte sem despedida e perdas múltiplas figuram como centrais no tocante ao luto.
