Correlation of motor function and respiratory individuals with Duchenne muscular dystrophy

Resumo: Since the gradual weakness of the respiratory muscles, especially the diaphragm, induces respiratory complications, monitoring the lung function of people with Duchenne Muscular Dystrophy (DMD) is essential. It is noteworthy that this deficiency in the respiratory system is the leading cause of death in patients with DMD. The objective was to correlate the motor and respiratory function of individuals with DMD. 14 individuals were evaluated, 09 being subdivided in the study group and 05 in the control group; all male, aged 11.3 (± 0.5) years, were evaluated using the scales for measuring motor function (MFM), Elgen Klassifikation (EK) and spirometry and manovacuometry tests. The MFM, EK, spirometry and manovacuometry variables were analyzed using the t-student test (p<0.05). Significant values were considered between motor function and spirometric variables and respiratory pressures analyzed using the Pearson correlation index. A significant reduction in motor function and greater functional dependence has been demonstrated in individuals with DMD. Spirometric and manovacuometric evaluations showed a reduction in lung function, and the correlations were significant, showing a strong relationship between the decline in motor function concomitant with lung function. The results of this study indicate that the impairment of motor function predicts the intensity of respiratory changes. It can be concluded that, in individuals with DMD, motor and respiratory function are reduced with advancing age; and that functional performance and respiratory conditions are directly associated. It is suggested that respiratory treatment should be combined with functional performance treatment.

Es de destacar que esta deficiencia en el sistema respiratorio es la principal causa de muerte en pacientes con DMD. El objetivo fue correlacionar la función motora y respiratoria de individuos con DMD. Se evaluaron 14 individuos, siendo 09 subdivididos en el grupo estudio y 05 en el grupo control; Todos los hombres, de 11,3 (± 0,5) años, fueron evaluados mediante las escalas de medición de la función motora (MFM), Elgen Klassifikation (EK) y pruebas de espirometría y manovacuometría. Las variables MFM, EK, espirometría y manovacuometría se analizaron mediante la prueba t-student (p <0,05). Se consideraron valores significativos entre la función motora y las variables espirométricas y las presiones respiratorias analizadas mediante el índice de correlación de Pearson. Se ha demostrado una reducción significativa de la función motora y una mayor dependencia funcional en personas con DMD. Las evaluaciones espirométricas y manovacuométricas mostraron una reducción en la función pulmonar, y las correlaciones fueron significativas, mostrando una fuerte relación entre la disminución de la función motora concomitante con la función pulmonar. Los resultados de este estudio indican que el deterioro de la función motora predice la intensidad de los cambios respiratorios. Se puede concluir que, en individuos con DMD, la función motora y respiratoria se reducen con el avance de la edad; y que el rendimiento funcional y las condiciones respiratorias están directamente asociados. Se sugiere combinar el tratamiento respiratorio con el tratamiento del rendimiento funcional.Uma vez que a fraqueza gradual dos músculos respiratórios, principalmente o diafragma, induz  complicações respiratórias, o acompanhamento da função pulmonar de pessoas com Distrofia Muscular de Duchenne (DMD) é primordial. Vale destacar que essa deficiência no sistema respiratório é a maior causa de morte de pacientes com DMD. Objetivou-se correlacionar a função motora e respiratória de indivíduos com DMD. Foram avaliados 14 indivíduos sendo subdivididos 09 no grupo estudo e 05 no grupo controle; todos do gênero masculino, com faixa etária de 11,3 (± 0,5) anos. A avaliação foi realizada através das escalas para medida de função motora (MFM), Elgen Klassifikation (EK) e exames de espirometria e manovacuometria. As variáveis da MFM, EK, espirometria e manovacuometria foram analisadas por meio do teste t - student (p<0.05). Foram considerados valores significativos entre função motora e variáveis espirométricas e pressões respiratórias analisados através do índice de correlação de Pearson. Foi demonstrada significativa redução da função motora e maior dependência funcional de indivíduos com DMD. As avaliações espirométricas e manovacuométricas evidenciaram redução da função pulmonar, e as correlações foram significativas, evidenciando forte relação entre o declínio da função motora concomitante à função pulmonar. Os resultados deste estudo apontam que o comprometimento da função motora prediz a intensidade das alterações respiratórias. Pode-se concluir que, nos indivíduos com DMD a função motora e respiratória são reduzidas de acordo com o avanço da idade; e que o desempenho funcional e as condições respiratórias estão diretamente associados.
