Cancer and Cachexia: influence of a therapeutic exercise protocol on the health and disease process

Resumo: Cancer is a clinical condition resulting from disordered cell proliferation, and has its association with cachexia, in part of the cases, this condition ends up worsening the patient's condition, due to the intense depletion of lean mass and adipose tissue, which causes immunological changes that compromise the effectiveness of drug therapy. Objective: This study aimed to evaluate the therapeutic effects of the application of resistance exercises of moderate intensity in cachectic patients with cancer of the gastrointestinal tract. Methodology: 20 healthy and sedentary individuals were selected for the control group, with an age range similar to the test group, which consisted of 20 cancer patients with gastrointestinal cancer, associated with cachexia, and the evaluations were performed with the following instruments: load cell, nutritional analysis of the menu, total energy value (VET), basal metabolic rate (BMR). The menu and motor control were evaluated before and after the resistance exercise protocol, compared to a group of normal people without association with the pathology and after the exercise protocol, the group of patients demonstrated an increase and strength control that approached the healthy group, being statistically significant Results: Some patients did not have the adequate caloric intake both when comparing the basal metabolic rate and when adding an exercise practice, visualized through the calculation of the total energy value, about the strength assessment, there was a significant improvement between the first and second evaluation, although the protocol time was not enough for lean mass production. Conclusion: The results found suggest that this treatment should be applied for longer than the one developed to assess whether the caloric intake was adequate, and if the proposed protocol would be effective for the production of muscle mass in these patients who have severe catabolic processes, evaluating the biological individuality of each patient, thus meeting the metabolic needs.

Objetivo: Este estudio tuvo como objetivo evaluar los efectos terapéuticos de la aplicación de ejercicios de resistencia de intensidad moderada en pacientes caquécticos con cáncer del tracto gastrointestinal. Metodología: se seleccionaron 20 individuos sanos y sedentarios para el grupo control, con rango de edad similar al grupo de prueba, el cual estuvo conformado por 20 pacientes oncológicos con cáncer gastrointestinal, asociado a caquexia, y las evaluaciones se realizaron con los siguientes instrumentos: celda de carga, análisis nutricional del menú, valor energético total (VET), tasa metabólica basal (TMB). Se evaluó el menú y control motor antes y después del protocolo de ejercicios de fuerza, en comparación con un grupo de personas normales sin asociación con la patología y después del protocolo de ejercicio, el grupo de pacientes demostró un aumento y control de la fuerza que se acercó a la grupo sano, siendo estadísticamente significativo Resultados: Algunos pacientes no tuvieron el aporte calórico adecuado tanto al comparar la tasa metabólica basal como al agregar una práctica de ejercicio, visualizada a través del cálculo del valor energético total, sobre la valoración de la fuerza, hubo una mejora significativa entre la primera y la segunda evaluación, aunque el tiempo del protocolo no fue suficiente para la producción en masa ajustada. Conclusión: Los resultados encontrados sugieren que este tratamiento debería ser aplicado por más tiempo que el desarrollado para evaluar si el aporte calórico fue adecuado y si el protocolo propuesto sería efectivo para la producción de masa muscular en estos pacientes que presentan procesos catabólicos severos, evaluando la individualidad biológica de cada paciente, satisfaciendo así las necesidades metabólicas.O câncer é uma condição clínica decorrente da proliferação celular desordenada, e tem sua associação com a caquexia, em parte dos casos, essa condição acaba por agravar o quadro do paciente, devido à intensa depleção da massa magra e do tecido adiposo, o que ocasiona alterações imunológicas que comprometem a eficácia da terapia medicamentosa. Objetivo: Este estudo teve como objetivo avaliar os efeitos terapêuticos da aplicação de exercícios resistidos de intensidade moderada em pacientes caquéticos com câncer do trato gastrointestinal. Metodologia: Foram selecionados 20 indivíduos saudáveis ​​e sedentários para o grupo controle, com faixa etária similar ao grupo teste que foi composta por 20 pacientes oncológicos com câncer gastrointestinal, associados à caquexia, e as avaliações foram realizadas com os seguintes instrumentos: célula de carga, análise nutricional do cardápio, valor energético total (VET), taxa metabólica basal (TMB).
