Analysis of the challenges of care management in field hospitals to face the COVID-19 pandemic: a narrative literature review

Resumo: | Introduction: The COVID-19 pandemic impacted the Management of Hospital Services, as well as the performance and role of leaders in the face of this new epidemic. Objective: to assess the prevalence of factors associated with the leadership role of hospital managers before and during the COVID-19 pandemic. Methodology: The research was carried out through online access to the National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Library (BVS) databases. and EBSCO Information Services, in March 2022. Results and discussion: COVID-19 evidenced a global crisis, requiring a transformation in the structure and work process of health units, highlighting the need for good planning for the face this epidemic advent. The research highlights the fundamental role of leadership in management to obtain good results: the hospital administrator must be assertive in decision-making and have good internal and external communication. In addition to training in the use of PPE, due to patients with severe and variable conditions, the assistance provided has become more complex, requiring knowledge that was not so required before, such as intubation, cricothyroidotomy and physical recovery processes, with managers responsible for the training of professionals. Final considerations: It is concluded that, far beyond routine actions, it is necessary to have specialized training, innovative vision and willingness to face potential challenges and abrupt adaptations, in the face of epidemic and pandemic episodes. |

Metodología: La investigación se realizó a través del acceso en línea a las bases de datos de la Biblioteca Nacional de Medicina (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Library (BVS) y Servicios de Información de EBSCO, en marzo de 2022. Resultados y discusión: El COVID-19 evidenció una crisis global, requiriendo una transformación en la estructura y proceso de trabajo de las unidades de salud, destacando la necesidad de una buena planificación para enfrentar este advenimiento epidémico. La investigación destaca el papel fundamental del liderazgo en la gestión para obtener buenos resultados: el administrador del hospital debe ser asertivo en la toma de decisiones y tener una buena comunicación interna y externa. Además de la capacitación en el uso de EPP, debido a pacientes con condiciones severas y variables, la asistencia brindada se ha vuelto más compleja, requiriendo conocimientos que antes no eran tan requeridos, como intubación, cricotiroidotomía y procesos de recuperación física, siendo los encargados responsables de la formación de profesionales. Consideraciones finales: Se concluye que, mucho más allá de las acciones rutinarias, es necesario contar con formación especializada, visión innovadora y disposición para enfrentar potenciales desafíos y adaptaciones abruptas, ante episodios epidémicos y pandémicos.Introdução: A pandemia COVID-19 impactou a Gestão dos Serviços Hospitalares, bem como o desempenho e o papel das lideranças frente a essa nova epidemia. Objetivo: avaliar a prevalência de fatores associados ao papel da liderança de gestores hospitalares antes e durante a pandemia de COVID-19. Metodologia: A pesquisa foi realizada através do acesso online nas bases de dados National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Biblioteca Virtual em Saúde (BVS) e EBSCO Information Services, no mês de março de 2022. Resultados e discussão: a COVID-19 evidenciou uma crise mundial, sendo necessária uma transformação na estrutura e processo de trabalho das unidades de saúde, em destaque a necessidade de um bom planejamento para o enfrentamento desse advento epidêmico. A pesquisa evidencia o papel fundamental da liderança na gestão para a obtenção de bons resultados: o administrador hospitalar deve ser assertivo na tomada de decisões e ter boa comunicação interna e externa. Além de capacitação em uso de EPI, devido a pacientes com quadros graves e variáveis, a assistência dos atendimentos se tornou mais complexa, necessitando de conhecimentos antes não tão exigidos, como intubação, cricotireoidostomia e processos de recuperação física, cabendo aos gestores a responsabilidade do treinamento dos profissionais.
