Health a right of all? Impasses encountered by the LGBTQIA+ population in the scope of Primary Health Care

Resumo: | Despite the 1988 Federal Constitution guaranteeing the right to universal and equal access to the entire population, through the Unified Health System (SUS), accessibility in primary health care (PHC) for the LGBTQIA+ community is still a major challenge. In this way, the objective was to survey the literature on the impasses encountered by the LGBTQIA+ population within the scope of PHC. For this, a search for studies was carried out through the electronic databases: PubMed, SciELO, Google Scholar and VHL, using the descriptors: Primary care “Primary attention”, Access to services “Access to services” LGBTQIA+ population “LGBTQIA+ population” and Saúde “Health”, combined by the Boolean operator “AND”. Only articles available in full were selected, being published in Portuguese or English, found in the time period from 2009 to March 2022, with content related to the central objective of the research. From the data analysis, it was evidenced that prejudice and discrimination are frequent violence in the daily life of the LGBTQIA+ population, having significant damages in several aspects of their life, especially on their health in an integral way. It was possible to understand that there is an invisibility regarding the care of the population that does not correspond to gender standards, due to the intense prejudice and discrimination on the part of professionals who do not have a preparation, since their graduation, to understand the social determinants and the themes. involving gender identity and sexual orientation. Thus, the LGBTQIA+ population is excluded from public health services and, consequently, a low life expectancy. |

De esta forma, el objetivo fue relevar la literatura sobre los impasses encontrados por la población LGBTQIA+ en el ámbito de la APS. Para ello se realizó una búsqueda de estudios a través de las bases de datos electrónicas: PubMed, SciELO, Google Scholar y BVS, utilizando los descriptores: Atención primaria “Atención primaria”, Acceso a servicios “Acceso a servicios” Población LGBTQIA+ “Población LGBTQIA+” y Saúde “Saúde”, combinados por el operador booleano “AND”. Fueron seleccionados solo artículos disponibles en su totalidad, publicados en portugués o inglés, encontrados en el período de 2009 a marzo de 2022, con contenido relacionado con el objetivo central de la investigación. Del análisis de los datos se evidenció que el prejuicio y la discriminación son violencias frecuentes en el cotidiano de la población LGBTQIA+, ocasionando daños significativos en varios aspectos de su vida, especialmente en su salud de manera integral. Fue posible comprender que existe una invisibilización en cuanto a la atención de la población que no corresponde a los estándares de género, debido al intenso prejuicio y discriminación por parte de profesionales que no están preparados, desde la graduación, para comprender los determinantes sociales y los temas. relacionados con la identidad de género y la orientación sexual. Así, la población LGBTQIA+ se encuentra excluida de los servicios públicos de salud y, en consecuencia, tiene una baja expectativa de vida.Apesar da Constituição Federal de 1988 garantir o direito ao acesso universal e igualitário a toda a população, por meio do Sistema Único de Saúde (SUS), a acessibilidade na atenção primária a saúde (APS) para a comunidade LGBTQIA+ ainda é um grande desafio. Dessa forma, objetivou-se fazer um levantamento na literatura sobre os impasses encontrados pela população LGBTQIA+ no âmbito da APS. Para isto, foi realizado uma busca de estudos através das bases de dados eletrônicas: PubMed, SciELO, Google Scholar e BVS, utilizando os descritores: Atenção primária “Primary attention”, Acesso aos serviços “Access to services” População LGBTQIA+ “LGBTQIA+ population” e Saúde “Health”, combinados pelo operador booleano “AND”. Selecionou-se apenas artigos disponíveis na íntegra, sendo eles publicados no idioma português ou inglês, encontrados no período temporal de 2009 a março 2022, com conteúdo relativo ao objetivo central da pesquisa. A partir da análise dos dados, evidenciou-se que o preconceito e a discriminação são violências frequentes no cotidiano da população LGBTQIA+, possuindo prejuízos significativos em vários aspectos da sua vida, especialmente sobre sua saúde de forma integral. Foi possível compreender que há uma invisibilização quanto ao cuidado da população que não corresponde aos padrões de gênero, devido ao intenso preconceito e discriminação por parte dos profissionais que não possuem um preparo, desde a sua graduação, para a compreensão dos determinantes sociais e as temáticas envolvendo identidade de gênero e orientação sexual.
