Ludwig's angina: clinical aspects and therapeutic approaches

Resumo: Ludwig's angina (AL) is an infection of the submandibular space, first described by Wilhelm Frederick von Ludwig in 1836. The presence of dental caries, oral trauma, immunosuppression and the continuous use of psychoactive substances, such as alcohol and drugs of abuse, are predisposing factors for the emergence of this infection. Given the above, this study aimed to review the literature on Ludwig's angina, highlighting its clinical aspects and therapeutic approaches. For the construction of this article, a bibliographic survey was carried out in the databases SciVerse Scopus, Scientific Electronic Library Online (Scielo), U.S. National Library of Medicine (PUBMED) and ScienceDirect, using the Mendeley reference manager. LA is a rapidly progressing infection of the floor of the mouth, which can rapidly compromise the patient's airways. It occurs most commonly in those with poor dentition or immunosuppression. Patients may have a hardened or woody floor of the mouth with submandibular swelling. Trismus is a late and serious finding. Due to the threat of rapid airway compromise, emergency consultation with anesthesia and otolaryngology is vital, especially if a definitive airway is required. The first-line approach to airway intervention in the ER is flexible intubation endoscopy with preparation for a surgical airway. Broad-spectrum antibiotics, such as ampicillin-sulbactam, and surgical source control are critical in infection control. Emergency physicians must be aware of this emerging diagnosis, requiring rapid recognition and airway protection.

La presencia de caries dental, trauma oral, inmunosupresión y el uso continuo de sustancias psicoactivas, como el alcohol y las drogas de abuso, son factores predisponentes para la aparición de esta infección. Teniendo en cuenta lo anterior, este estudio tuvo como objetivo revisar la literatura sobre la angina de Ludwig, destacando sus aspectos clínicos y enfoques terapéuticos. Para la construcción de este artículo se realizó un levantamiento bibliográfico en las bases de datos SciVerse Scopus, Scientific Electronic Library Online (Scielo), Estados Unidos. Biblioteca Nacional de Medicina (PUBMED) y ScienceDirect, utilizando el gestor de referencias de Mendeley. LA es una infección del piso de la boca que progresa rápidamente y puede comprometer rápidamente las vías respiratorias del paciente. Ocurre con mayor frecuencia en personas con mala dentición o inmunosupresión. Los pacientes pueden tener un piso de la boca endurecido o leñoso con hinchazón submandibular. El trismo es un hallazgo tardío y grave. Debido a la amenaza de compromiso rápido de las vías respiratorias, la consulta de emergencia con anestesia y otorrinolaringología es vital, especialmente si se requiere una vía aérea definitiva. El enfoque de primera línea para la intervención de las vías respiratorias en la sala de emergencias es la endoscopia de intubación flexible con preparación para una vía aérea quirúrgica. Los antibióticos de amplio espectro, como la ampicilina-sulbactam, y el control quirúrgico de la fuente son críticos en el control de infecciones. Los médicos de urgencias deben ser conscientes de este diagnóstico emergente, que requiere un reconocimiento rápido y protección de las vías respiratorias.A angina de Ludwig (AL) é uma infecção do espaço submandibular, primeiramente descrita por Wilhelm Frederick von Ludwig em 1836. Diante do exposto, esse estudo objetivou revisar a literatura acerca da angina de Ludwig, ressaltando seus aspectos clínicos e abordagens terapêuticas.  Para a construção deste artigo foi feito um levantamento bibliográfico nas bases de dados SciVerse Scopus, Scientific Eletronic Library Online (Scielo), U.S. National Library of Medicine (PUBMED) e ScienceDirect, usando o gerenciador de referências Mendeley.
