Nursing assistance in pressure injuries in judged patients

Resumo: | The study aims to demonstrate the presence of pressure injuries and describe nursing care for bedridden patients who suffer from these injuries in a hospitalization unit. This is a qualitative, exploratory study, with a literature review. According to data collected, the bedridden patient is highly prone to develop pressure injuries, which can be caused by several factors, among them, when one of the areas of the body is pressed under some soft or rigid surface, with humidity and lack of blood circulation, however, injuries in general can be preventable. Thus, nursing together with the multidisciplinary team must be prepared to develop, develop and apply strategies in the prevention and treatment of patients, once they are aware of the location of the formation of the lesions, medical diagnosis and care provided, they will result in quality of life to the patient. The prescription and nursing care contribute a lot in the care process, the thorough application of prevention in the formation of pressure injuries, must be followed with specific conducts for each case, as well as with the necessary contribution in the conduct. Thus, the application of the Braden scale and the aggregation of strategies applied in prevention, are essential resources to identify the potential risk of the patient, in the evolution of the treatment effectiveness and in the aggravating factor of these injuries. It is concluded that with all the observances raised and with indications of accurate nursing attention, it is assumed that there is a great possibility that the patient will be able to achieve very soon, to recover from the painful pathological process. |

Se trata de un estudio cualitativo, exploratorio, con revisión de la literatura. Según los datos recogidos, el paciente encamado es muy propenso a desarrollar lesiones por presión, las cuales pueden ser provocadas por varios factores, entre ellos, cuando una de las zonas del cuerpo es presionada bajo alguna superficie blanda o rígida, con humedad y falta de sangre. circulación, sin embargo, las lesiones en general se pueden prevenir. Así, la enfermería junto con el equipo multidisciplinario deben estar preparados para desarrollar, desarrollar y aplicar estrategias en la prevención y tratamiento de los pacientes, una vez que conozcan la ubicación de la formación de las lesiones, el diagnóstico médico y los cuidados brindados, darán como resultado calidad de vida del paciente. La prescripción y los cuidados de enfermería aportan mucho en el proceso asistencial, la aplicación detallada de la prevención en la formación de lesiones por presión, debe seguirse con conductas específicas para cada caso, así como con el aporte necesario en la conducta. Así, la aplicación de la escala de Braden y la agregación de estrategias aplicadas en prevención, son recursos fundamentales para identificar el riesgo potencial del paciente, en la evolución de la efectividad del tratamiento y en el agravante de estas lesiones. Se concluye que con todas las observancias planteadas y con indicaciones de una atención de enfermería precisa, se asume que existe una gran posibilidad de que el paciente pueda lograr muy pronto, recuperarse del proceso patológico doloroso.O estudo objetiva demonstrar a presença das lesões por pressão e descrever a assistência da enfermagem aos pacientes acamados que sofrem dessas lesões em unidade de internação hospitalar. Trata-se de um estudo qualitativo, exploratório, com revisão de literatura. Segundo dados levantados, o paciente acamado encontra-se com grande propensão a desenvolver lesões por pressão, que podem ser ocasionadas por vários fatores, dentre eles, quando uma das áreas do corpo se encontra pressionada sob alguma superfície macia ou rígida, com umidade e falta de circulação sanguínea, entretanto, as lesões de uma maneira geral, podem ser preveníveis. Assim, a enfermagem juntamente com a equipe multiprofissional deve estar preparada para elaborar, desenvolver e aplicar estratégias na prevenção e tratamento dos pacientes, uma vez estando atentos quanto a localização da formação das lesões, diagnóstico médico e cuidados prestados, resultarão em qualidade de vida ao paciente. A prescrição e os cuidados da enfermagem contribuem muito no processo do cuidar, a aplicação minuciosa de prevenção na formação das lesões por pressão, deve ser seguida com condutas específicas para cada caso, assim como, com o aporte necessário na conduta. Assim, a aplicação da escala de Braden e a agregação de estratégias aplicadas na prevenção, são recursos primordiais para identificar o potencial risco do paciente, na evolução da eficácia do tratamento e no fator de agrave dessas lesões.
