Pain as the 5th vital sign and nursing records: an integrative review

Resumo: As it is a prevalent symptom in the report of most patients, today, pain is a mandatory assessment criterion. The inclusion of pain as the 5th vital sign, with appropriate registration and consequent intervention, ensures that all patients have access to effective measures for pain control. Considering the proximity and the time that the nursing team remains with the patients, it is necessary to promote permanent education directed to these professionals so that, in fact, the systematic assessment of pain occurs. The objective is to describe how pain is recorded as the 5th vital sign in the care of hospitalized patients. It is an integrative review with six articles selected from the Lilacs, PubMed and SciELO databases between 2010 and 2020. After reading, analyzing and interpreting the data, the publications showed that the practice of pain records as 5 The vital sign should be more emphasized, aiming to optimize care and positively influence the patient's health and quality of life. Thus, the research reflects on the need to provide training to nursing professionals for adequate pain management in hospitalized patients.

La inclusión del dolor como quinto signo vital, con el registro adecuado y la consiguiente intervención, asegura que todos los pacientes tengan acceso a medidas efectivas para el control del dolor. Considerando la proximidad y el tiempo que el equipo de enfermería permanece con los pacientes, es necesario promover la educación permanente dirigida a estos profesionales para que, de hecho, se produzca la evaluación sistemática del dolor. El objetivo es describir cómo el dolor se registra como el quinto signo vital en la atención de pacientes hospitalizados. Se trata de una revisión integradora con seis artículos seleccionados de las bases de datos Lilacs, PubMed y SciELO entre 2010 y 2020. Considerando a proximidade e o tempo em que a equipe de enfermagem permanece com os pacientes, é necessário promover educação permanente direcionada a estes profissionais para que, de fato, ocorra a avaliação sistematizada da dor. Objetiva-se descrever como ocorre o registro da dor como o 5º sinal vital no cuidado ao paciente hospitalizado. Trata-se de uma revisão integrativa com seis artigos selecionados nas bases de dados Lilacs, PubMed e SciELO entre o período de 2010 e 2020. Após a leitura, análise e interpretação dos dados, as publicações evidenciaram que a prática de registros de dor como 5° sinal vital deve ser mais enfatizada, visando otimizar o atendimento e influenciar na saúde e qualidade de vida do paciente de forma positiva.
