Hemoglobinopathies profile in pregnant women: spatial distribution and temporal analysis from 2013 to 2019 in the State of Piauí, Brazil

Resumo: | This article aims to identify the profile of hemoglobinopathies in pregnant women diagnosed during prenatal care in the State of Piauí through spatial distribution and temporal analysis based on data from a public health laboratory. The study analyzed the results of pregnant patients at the Piauí Central Public Health Laboratory from January 2013 to December 2019. In all, 39,616 hemoglobinopathy research exams were performed; 2,480 (6.26%) of the pregnant women tested positive for hemoglobinopathies. There was a prevalence of pregnant women with Hb AS (73.63%) and Hb AC (10.56%); the sickle cell disease genotypes were Hb SC: 5.48%, Hb SF: 0.77% and Hb SS: 0.44%; less prevalent variant hemoglobins: Hb AE (0.12%), Hb AF (0.08%) and Hb CC (0.04%). Most of the pregnant women identified with hemoglobinopathies were aged between 13 and 25 years and were located in the central-northern piauiense mesoregion. The temporal analysis revealed slight percentage changes over the years and obtained a higher rate in 2018 with 6.7% of pregnant women diagnosed. The spatial distribution identified variant hemoglobins in 56 municipalities, among them, Teresina, União, José de Freitas, Bom Jesus, Oeiras and Valença do Piauí had the highest number of cases. The results obtained indicate the potential risk of sickle cell trait and hemoglobin C trait among pregnant women in different regions of Piauí, increasing the probability of newborns with the disease, whether homozygous or heterozygous, reflecting an important public health issue in the State. |

El estudio analizó los resultados de pacientes embarazadas en el Laboratorio Central de Salud Pública de Piauí desde enero de 2013 hasta diciembre de 2019. En total, se realizaron 39,616 exámenes de investigación de hemoglobinopatía; 2.480 (6,26%) de las mujeres embarazadas dieron positivo por hemoglobinopatías. Hubo una prevalencia de gestantes con Hb AS (73,63%) y Hb AC (10,56%); los genotipos de anemia falciforme fueron Hb SC: 5,48%, Hb SF: 0,77% y Hb SS: 0,44%; variantes de hemoglobinas menos prevalentes: Hb AE (0,12%), Hb AF (0,08%) y Hb CC (0,04%). La mayoría de las embarazadas identificadas con hemoglobinopatías tenían entre 13 y 25 años y estaban ubicadas en la mesorregión centro-norte del piauiense. El análisis temporal reveló ligeros cambios porcentuales a lo largo de los años y obtuvo una tasa mayor en 2018 con un 6,7% de gestantes diagnosticadas. La distribución espacial identificó hemoglobinas variantes en 56 municipios, entre ellos, Teresina, União, José de Freitas, Bom Jesus, Oeiras y Valença do Piauí tuvieron el mayor número de casos. Los resultados obtenidos indican el riesgo potencial del rasgo drepanocítico y el rasgo de hemoglobina C entre gestantes de diferentes regiones de Piauí, aumentando la probabilidad de recién nacidos con la enfermedad, ya sean homocigotos o heterocigotos, reflejando un importante problema de salud pública en el Estado.Este artigo tem como objetivo identificar o perfil de hemoglobinopatias em gestantes diagnosticadas durante o pré-natal no Estado do Piauí através de uma distribuição espacial e análise temporal com base nos dados de um laboratório de saúde pública. O estudo analisou os resultados de pacientes gestantes do Laboratório Central de Saúde Pública do Piauí no período entre janeiro de 2013 a dezembro de 2019. Ao todo 39.616 exames de pesquisa de hemoglobinopatias foram realizados; 2.480 (6,26%) das gestantes testaram positivo para hemoglobinopatias. Verificou-se prevalência de gestantes portadoras de Hb AS (73,63%) e Hb AC (10,56%); já os genótipos de doenças falciformes foram Hb SC: 5,48% , Hb SF: 0,77% e Hb SS: 0,44%; hemoglobinas variantes menos prevalentes: Hb AE (0,12%), Hb AF (0,08%) e Hb CC (0,04%). A maioria das gestantes identificadas com hemoglobinopatias tinham idade entre 13 a 25 anos e localizavam-se na mesorregião Centro-Norte Piauiense. A análise temporal revelou alterações percentuais discretas ao longo dos anos e obteve maior índice no ano de 2018 com 6,7% de gestantes diagnosticadas. A distribuição espacial identificou hemoglobinas variantes em 56 municípios, entre eles, Teresina, União, José de Freitas, Bom Jesus, Oeiras e Valença do Piauí apresentaram maior quantidade de casos.
