Isokinetic evaluation of the knee in adults with cerebral palsy: a literature review

Resumo: Objective: To verify the profile of adults with cerebral palsy (CP) included in the studies and the parameters most utilized in the isokinetic evaluation of the muscles that move the knee. Method: Bibliografh rsearches were carried out in the databases: PubMed, LILACS, Cochrane Central Register of Controlled Trials (CENTRAL), Physiotherapy Evidence Database (PEDro) e SciELO, articles published from the beginning of each database until November 2021 were searched. The search terms used were “cerebral palsy”; "adult"; “isokinetic” or “isokinetic”; “dynamometer” or “dynamometry”; “joelho”, in Portuguese and English. Results: A total of 69 articles were found in the initial search (PubMed= 34, LILACS=12, Cochrane CENTRAL= 8, PEDro= 9 and SciELO= 6. After removing the duplicates, 46 studies remained. In the first screening by title and abstract 11 studies were selected. After reading these 11 articles in full and applying the eligibility criteria, a total of seven articles remained. Conclusion: Most studies included people of both sexes, followed by men, with bilateral CP or spastic diplegia and unilateral CP or hemiparesis. Most studies included soccer athletes with CP and did not include control group. Most used the Cybex 6000 isokinetic dynamometer, evaluated the knee extensor and flexor muscles, used the concentric contraction type and 60o/s angular velocity. Peak torque was the most studied variable and the position of the individuals, the alignment of the dynamometer axis and the lever arm were described in most studies.

Metodología: Se realizaron levantamientos bibliográficos en las siguientes bases de datos: PubMed, LILACS, Cochrane Central Register of Controlled Trials (CENTRAL), Physiotherapy Evidence Database (PEDro) y SciELO, se buscaron artículos publicados desde el inicio de cada base de datos hasta noviembre de 2021. Los términos de búsqueda utilizados fueron “parálisis cerebral”; "adulto"; “isocinético” o “isocinético”; “dinamómetro” o “dinamometría”; “joelho”, en portugués e inglés. Resultados: En la búsqueda inicial se encontraron un total de 69 artículos (PubMed= 34, LILACS=12, Cochrane CENTRAL= 8, PEDro= 9 y SciELO= 6. Después de eliminar los duplicados quedaron 46 estudios. En la primera selección por título y se seleccionaron 11 estudios abstractos. Después de leer estos 11 artículos en su totalidad y aplicar los criterios de elegibilidad, quedaron un total de siete artículos. Conclusión: La mayoría de los estudios incluyeron personas de ambos sexos, seguidos de hombres, con PC bilateral o diplejía espástica y PC unilateral o hemiparesia. La mayoría de los estudios incluyeron atletas de fútbol con parálisis cerebral y no incluyeron un grupo de control. La mayoría utilizó el dinamómetro isocinético Cybex 6000, evaluó los músculos extensores y flexores de la rodilla, utilizó el tipo de contracción concéntrica y velocidad angular de 60o/s. El torque pico fue la variable más estudiada y la posición de los individuos, la alineación del eje del dinamómetro y el brazo de palanca fueron descritos en la mayoría de los estudios.Objetivo: Verificar o perfil dos adultos com paralisia cerebral (PC) incluídos nos estudos e os parâmetros maisutilizados na avaliação isocinética dos músculos que movem o joelho. Metodologia: Foram realizados levantamentos bibliográficos nas bases de dados: PubMed, LILACS, Cochrane Central Register of Controlled Trials (CENTRAL), Physiotherapy Evidence Database (PEDro) e SciELO, foram pesquisados artigos publicados desde o início de cada base de dados até novembro de 2021. Os termos utilizados para a busca foram “paralisia cerebral”; “adulto”; “isocinético” ou “isocinética”; “dinamômetro” ou “dinamometria”; “joelho”, em português e inglês. Resultados: Foi encontrado o total de 69 artigos na busca inicial (PubMed= 34, LILACS=12, Cochrane CENTRAL= 8, PEDro= 9 e SciELO= 6. Após a remoção das duplicatas permaneceram 46 estudos. Na primeira triagem pelo título e resumo foram selecionados 11 estudos. Após a leitura destes 11 artigos na íntegra e aplicados os critérios de elegibilidade restaram um total de sete artigos. Conclusão: A maioria dos estudos incluiu pessoas de ambos os sexos, seguido de pessoas do sexo masculino, com PC bilateral ou diplegia espástica e PC unilateral ou hemiparesia.  A maioria dos estudos incluiu atletas de futebol com PC e não incluiu grupo controle. A maioria utilizou o dinamômetro isocinético Cybex 6000, avaliou a musculatura extensora e flexora do joelho, utilizou o tipo de contração concêntrica e a velocidade angular de 60o/s.
