Mandibular osteonecrosis and the use of bisphosphonate in an oncologic patient: case report

Resumo: Clinical case report of a patient who remains under medical oncology care due to bilateral malignant neoplasm of the breast with bilateral bone and lung metastasis, who developed difficult-to-manage mandibular osteonecrosis related to prosthetic oral trauma and the use of injectable bisphosphonates. This study was carried out in the dentistry service of the Oncology Center of the Oswaldo Cruz University Hospital of the University of Pernambuco. It was possible to take care of, favor and monitor the self-resolving of mandibular bone necrosis for two years, providing oral comfort and the absence of infection, without interruption of hormonal antineoplastic treatment; regardless of the initially proposed peripheral osteotomy procedure to be performed under general anesthesia, it was refused by the patient and family. The autonomy of the patient was respected, and thus, the Standard Operating Protocol for Oral Care (POP-Oral), by Vidal, 2012, associated with antibiotic and analgesic therapy, for three months, and then remained only with POP-Oral and biweekly applications 0.2% chlorhexidine gel until spontaneous expulsion of necrotic bone and excellent healing occurred. Currently, the patient remains under follow-up, remaining without symptoms or any other oral alteration. It is concluded that the treatment of osteonecrosis of the jaws is of great complexity, and there is still no specific treatment, but it is possible to contain, avoid infections and stabilize the evolution of bone lesions, with individualized care, a multidisciplinary team and the exercise of bioethics, in medical-dental practice, as can be seen in this report.

Este estudio fue realizado en el servicio de odontología del Centro de Oncología del Hospital Universitario Oswaldo Cruz de la Universidad de Pernambuco. Se logró cuidar, favorecer y monitorear la auto-resolución de la necrosis ósea mandibular durante dos años, brindando confort bucal y ausencia de infección, sin interrupción del tratamiento hormonal antineoplásico; independientemente del procedimiento de osteotomía periférica propuesto inicialmente para ser realizado bajo anestesia general, el mismo fue rechazado por el paciente y la familia. Se respetó la autonomía del paciente, y así, el Protocolo Operativo Estándar de Cuidado Bucal (POP-Oral), de Vidal, 2012, asociado a terapia antibiótica y analgésica, durante tres meses, y luego se mantuvo solo con POP-Oral y quincenalmente. aplicaciones de gel de clorhexidina 0,2% hasta que se produjo la expulsión espontánea del hueso necrótico y una excelente cicatrización. Actualmente el paciente permanece en seguimiento permaneciendo sin síntomas ni ninguna otra alteración bucal. Se concluye que el tratamiento de la osteonecrosis de los maxilares es de gran complejidad, y aún no existe un tratamiento específico, pero es posible contener, evitar infecciones y estabilizar la evolución de las lesiones óseas, con atención individualizada, un equipo multidisciplinario y la ejercicio de la bioética, en la práctica médico-odontológica, como se puede apreciar en este informe.Relato de caso clínico de uma paciente que segue sob assistência médica oncológica em decorrência de neoplasia maligna bilateral da mama com metástase óssea e pulmonar bilateral, a qual desenvolveu osteonecrose mandibular de difícil manejo, relacionada a trauma oral protético e ao uso de bisfosfonato injetável. Esse estudo foi vivenciado no serviço de odontologia do Centro de Oncologia do Hospital Universitário Oswaldo Cruz da Universidade de Pernambuco. Foi possível cuidar, favorecer e acompanhar a auto-resolução da necrose óssea mandibular durante dois anos, propiciando o conforto oral e a ausência de infecção, sem interrupção do tratamento antineoplásico hormonal; independentemente do procedimento inicialmente proposto de osteotomia periférica a ser realizado sob anestesia geral, ter sido recusado pela paciente e familiares. Foi respeitada a autonomia da paciente, e assim, foi utilizado o Protocolo Operacional Padrão de Cuidados Orais (POP-Oral), de Vidal, 2012, associado a terapia antibiótica e analgésica, por três meses, e depois permaneceu só com o POP-Oral e aplicações quinzenais de gel de clorexidina 0,2% até que ocorreu a expulsão espontânea do osso necrótico e excelente cicatrização.
