Congenital syphilis: Incidence and epidemiological characterization in an Amazon state

Resumo: Objectives: To describe the incidence behavior and define the clinical, epidemiological and laboratory profile of congenital syphilis in the state of Amapá, Brazilian Amazon, from 2014 to 2018.Methods: A descriptive and cross-sectional study was conducted based on data provided by the Health Surveillance Secretariat of the estate of Amapá. The annual incidence rate for 1000 live births was calculated and its spatial distribution in the state was described. In addition, descriptive statistics of sociodemographic, clinical and laboratory variables, and inferential association statistics were performed, using Fisher's Exact Test, for the relevant categorical variables. Results: There were registered 308 cases of congenital syphilis, with increasing incidence of the disease, and with Macapá city, the capital, representing 71.1% of the cases. In the profile of new borns, there was a predominance of race / brown color (47.37%), recent congenital syphilis (99.68%), and jaundice as the main symptom (68.5%). Regarding the maternal profile, it was characterized by women under 20 years old (31.35%), brown (76.3%), with incomplete 5th to 8th-grade education (25.97%), and housewives (82.94%). It was observed that prenatal care was performed in 72.4% of cases, but with a diagnosis prior to delivery in only 36.04%, the partner was not treated in 72.4%, and maternal treatment was inadequate in 63.3%.Conclusions: It was demonstrated an increasing behavior of the disease incidence, with a characteristic profile of involvement. It is essential to carry out and register diagnostic tests, as well as the solidification of prenatal services and continued training of health professionals.

Métodos: Se realizó un estudio descriptivo y transversal con base en datos proporcionados por la Secretaría de Vigilancia en Salud del Estado de Amapá. Se calculó la tasa de incidencia anual para 1000 nacidos vivos y se describió su distribución espacial en el estado. Además, se realizó estadística descriptiva de variables sociodemográficas, clínicas y de laboratorio, y estadística de asociación inferencial, mediante el Test Exacto de Fisher, para las variables categóricas relevantes. Resultados: se registraron 308 casos de sífilis congénita, con comportamiento creciente de la incidencia de la enfermedad, y el municipio de Macapá, capital, representó el 71,1% de los casos. En el perfil de los recién nacidos, predominó la raza / color marrón (47,37%), la sífilis congénita reciente (99,68%) y la ictericia como síntoma principal (68,5%). En cuanto al perfil materno, se caracterizó por mujeres menores de 20 años (31,35%), morenas (76,3%), con escolaridad completa de 5º a 8º grado (25,97%) y amas de casa (82,94%). Se observó que la atención prenatal se realizó en el 72,4% de los casos, pero con un diagnóstico previo al parto solo en el 36,04%, la pareja no fue atendida en el 72,4% y el tratamiento materno fue inadecuado en el 63,3%. Conclusiones: Muestra un comportamiento creciente de la incidencia de la enfermedad, con un perfil de afectación característico. Es fundamental la realización y registro de pruebas y exámenes diagnósticos, así como la solidificación de los servicios prenatales y la formación continua de los profesionales de la salud.Objetivos: Descrever o comportamento da incidência e delimitar o perfil clínico, epidemiológico e laboratorial da sífilis congênita no estado do Amapá, Amazônia brasileira, de 2014 a 2018. No perfil dos recém-nascidos, houve predomínio da raça/cor parda (47,37%), da sífilis congênita recente (99,68%), e da icterícia como principal sintoma (68,5%). No que tange o perfil materno, caracterizou-se por mulheres menores de 20 anos (31,35%), pardas (76,3%), com escolaridade de 5ª a 8ª série completas (25,97%), e donas de casa (82,94%). Observou-se que o pré-natal foi realizado em 72,4% dos casos, porém com diagnóstico anterior ao parto em somente 36,04%, o parceiro não foi tratado em 72,4%, e o tratamento materno foi inadequado em 63,3%. Conclusões: Demonstra comportamento crescente da incidência da doença, com perfil característico de acometimento.
