Cardiovascular risk scores among workers assisted at a Workers' Health Reference Center

Resumo: This study seeks to identify the risk of developing a cardiovascular disease in ten years among workers assisted at a Reference Center for Occupational Health. For that purpose, the Risk Score developed by the World Health Organization and the International Society of Hypertension (WHO/ISH) was applied. This is a longitudinal, retrospective, dynamic population study, carried out with secondary data obtained from a nutrition clinic located in the city of Salvador, which serves workers with occupational disease in the state of Bahia, Brazil. Risk Score calculation is based on known risk factors for cardiovascular disease, gender, age, presence or absence of diabetes, in smokers or non-smokers alike, systolic blood pressure and total cholesterol levels. For ease of presentation and understanding, the Risk Score was analyzed as: a) low - less than 10% risk; b) moderate/high – greater than or equal to 10%. Results show that, in the first consultation, workers showed evidence of moderate/high risk for CVD among women aged over 50 years, with alcohol and tobacco consumption habits, with over 9 years of education and who did not practice physical activity. Regarding anthropometric, biochemical and blood pressure characteristics, obese individuals with total cholesterol, triglycerides, systolic pressure and abdominal perimeter above normal parameters are in the moderate/high group for cardiovascular disease. It is concluded that appropriate interventions capable of addressing the various social, cultural and emotional aspects, among others, are necessary, which can increase the workers' level of information, helping them to improve their health.

Por lo tanto, se utilizó el Risk Score propuesto por la Organización Mundial de la Salud y la Sociedad Internacional de Hipertensión (OMS/SIH). Se trata de un estudio longitudinal, retrospectivo, dinámico, poblacional, realizado con datos secundarios obtenidos de una clínica de nutrición ubicada en la ciudad de Salvador, que atiende a trabajadores con enfermedad profesional en el estado de Bahia, Brasil. El cálculo de Risk Score se basa en factores de riesgo conocidos de enfermedad cardiovascular, sexo, edad, presencia o ausencia de diabetes, si fuma o no, presión arterial sistólica y niveles de colesterol total. Para facilitar la presentación y la comprensión, el Risk Score se analizó como: a) bajo: menos del 10% de riesgo; b) moderado/alto – mayor o igual al 10%. Los resultados muestran que, en la primera consulta, los trabajadores evidenciaron riesgo moderado/alto para ECV entre mujeres mayores de 50 años, con hábitos de consumo de alcohol y tabaco, con más de 9 años de educación y que no practicaban actividad física. En cuanto a las características antropométricas, bioquímicas y de presión arterial, los obesos con colesterol total, triglicéridos, presión sistólica y perímetro abdominal por encima de los parámetros normales se encuentran en el grupo moderado/alto de enfermedad cardiovascular. Se concluye que son necesarias intervenciones adecuadas capaces de atender los diversos aspectos sociales, culturales y emocionales, entre otros, que puedan aumentar el nivel de información de los trabajadores, ayudándoles a promover su salud.Este estudo objetiva identificar o risco de desenvolver uma doença cardiovascular em dez anos entre trabalhadores atendidos em um Centro de Referência em Saúde do Trabalhador, utilizando o RISK SCORE proposto pela  Organização Mundial da Saúde e Sociedade Internacional de Hipertensão (OMS/ISH). Trata-se de um estudo longitudinal, retrospectivo, de população dinâmica, realizado com dados secundários obtidos em um ambulatório de nutrição, localizado no município de Salvador, que atende trabalhadores com doença ocupacional do Estado da Bahia. O cálculo do Risk Score se baseia nos fatores de risco conhecidos para doenças cardiovasculares, sexo, idade, presença ou ausência de diabetes, se fumante ou não fumante, pressão arterial sistólica e níveis de colesterol total. Para facilidade de apresentação e compreensão o Risk Score foi analisado como: a) baixo - menos de 10% de risco; b) moderado/alto – maior ou igual a 10%.  Os resultados mostram que, na primeira consulta, os trabalhadores apresentaram evidências de risco moderado/alto para DCV entre as mulheres, com idade acima de 50 anos, com hábito de consumir álcool e tabaco, com mais de 9 anos de estudo e que não praticavam atividade física. Em relação as características antropométricas, bioquímicas e de pressão arterial, obesos, com colesterol total, triglicérides, pressão sistólica e perímetro abdominal acima dos parâmetros da normalidade estão no grupo moderado/alto para doença cardiovascular.
