Lymphocytic vasculitis and oral lesions as exclusive manifestations of the late phase of Covid-19: Case report in an elderly woman

Resumo: Objective: SARS-CoV-2 infection has a wide spectrum of clinical manifestations, and skin lesions associated with infection are increasingly reported, simultaneously with respiratory symptoms or in isolation. Methodology: This is a case report with the purpose of describing and discussing the presented case, with a brief literature review. Results: In this case report, we present an elderly woman, not previously vaccinated for Covid-19, with purpuric lesions on the skin and with a moniliform appearance in the oral cavity, without symptoms or characteristic signs of respiratory infection and positive IgG serology for Covid-19. The histopathological examination of the skin showed vasculitis of small vessels, predominantly lymphocytic, restricted to the superficial dermis. There was laboratory exclusion of etiologies often causing skin lesions similar to the case. Cutaneous manifestations of Covid-19 are addressed, and lymphocytic vasculitis is discussed as a late manifestation of Covid-19. Conclusion: This case highlights the importance of identifying skin lesions associated with Covid-19, for taking appropriate measures to prevent viral transmission.

Metodología: Se trata de un caso clínico con el propósito de describir y discutir el caso presentado, con una breve revisión bibliográfica. Resultados: En este reporte de caso se presenta una mujer de edad avanzada, no vacunada previamente para Covid-19, con lesiones purpúricas en piel y de aspecto moniliforme en cavidad bucal, sin síntomas ni signos característicos de infección respiratoria y serología IgG positiva para Covid-19. El examen histopatológico de la piel mostró vasculitis de pequeños vasos, de predominio linfocitario, restringida a la dermis superficial. Hubo exclusión de laboratorio de etiologías que frecuentemente causaban lesiones cutáneas similares al caso. Se abordan las manifestaciones cutáneas de la Covid-19 y se discute la vasculitis linfocítica como manifestación tardía de la Covid-19. Conclusión: Este caso destaca la importancia de identificar las lesiones cutáneas asociadas al Covid-19, para tomar las medidas adecuadas para prevenir la transmisión viral.Objetivo: A infecção por SARS-CoV-2 tem amplo espectro de manifestações clínicas, sendo cada vez mais relatado lesões cutâneas associadas à infecção, de forma simultânea aos sintomas respiratórios ou isoladamente. Metodologia: Trata-se de um caso clínico com a finalidade de descrever e discutir o caso apresentado, com breve revisão da literatura. Resultados: Neste relato de caso, apresentamos uma idosa, não vacinada previamente para Covid-19, com lesões purpúricas na pele e de aspecto moniliforme na cavidade oral, sem sintomas ou sinais característicos de infecção respiratória e sorologia IgG positiva para Covid-19. O exame histopatológico da pele evidenciou vasculite de pequenos vasos predominantemente linfocitária, restrito à derme superficial. Houve exclusão laboratorial de etiologias frequentemente causadoras de lesões cutâneas similares ao caso. Manifestações cutâneas da Covid-19 são abordadas, e a vasculite linfocítica é discutida como manifestação tardia da Covid-19.
