Effects of Vitamin-D supplementation in sarcopenic elderly: an integrative review

Resumo: Introduction: Sarcopenia, initially defined as skeletal muscle loss, has gained importance and the study of this entity has shown the complexity involved in its pathophysiology, as well as its interaction with other metabolic pathways, such as vitamin-D. The loss of muscle mass increases in conjunction with advancing age, as the concentration of vitamin-D tends to be lower at advanced ages. The update on the interactions of vitamin-D on muscle function and its benefits on muscle health, especially in sarcopenic elderly people, is important to elucidate possible clinical approaches. Methodology: This is an integrative review, which sought to answer the effects of vitamin-D supplementation in sarcopenic elderly, in the following databases: Virtual Health Library (BVS), National Library of Medicine (PubMed MEDLINE) and Scientific Electronic Library Online (Scielo). Results and Discussion: Vitamin-D hypovitaminosis seems to directly influence the pathophysiology of sarcopenia and muscle function. Its supplementation seems to bring benefits such as increased muscle mass and strength and improvement in postural instability, which may influence the reduction of the risk of falls and mortality in elderly patients. However, a consensus has not yet been established, due to divergences in the variables studied. Conclusion: Evidence seems to converge to the idea that beneficial effects are observed mainly in those patients over 65 years of age and with vitamin-D levels below 30nmol/L, supplementing from 700 - 1000 IU/day.

La pérdida de masa muscular aumenta con la edad, ya que la concentración de vitamina D tiende a ser menor en edades avanzadas. La actualización sobre las interacciones de la vitamina D en la función muscular y sus beneficios en la salud muscular, especialmente en ancianos sarcopénicos, es importante para dilucidar posibles enfoques clínicos. Metodología: Se trata de una revisión integradora, que buscó responder los efectos de la suplementación con vitamina D en ancianos sarcopénicos, en las siguientes bases de datos: Virtual Health Library (BVS), National Library of Medicine (PubMed MEDLINE) y Scientific Electronic Library Online (Scielo). Resultados y Discusión: La hipovitaminosis por vitamina D parece influir directamente en la fisiopatología de la sarcopenia y la función muscular. Su suplementación parece traer beneficios como aumento de la masa y fuerza muscular y mejora de la inestabilidad postural, lo que puede influir en la reducción del riesgo de caídas y mortalidad en pacientes de edad avanzada. Sin embargo, aún no se ha establecido un consenso, debido a las divergencias en las variables estudiadas. Conclusión: La evidencia parece converger a la idea de que los efectos beneficiosos se observan principalmente en aquellos pacientes mayores de 65 años y con niveles de vitamina D por debajo de 30 nmol/L, suplementando de 700 a 1000 UI/día.Introdução: A sarcopenia, definida inicialmente como a perda muscular esquelética, tem ganhado importância e o estudo dessa entidade vem mostrando a complexidade envolvida na sua fisiopatologia, assim como sua interação com outras vias metabólicas, como a da vitamina-D. A perda de massa muscular aumenta em conjunto com o avanço da idade, assim como a concentração de vitamina-D tende a ser menor em idades avançadas. A atualização sobre as interações da vitamina-D na função muscular e seus benefícios acerca da saúde muscular, sobretudo de idosos sarcopênicos, é importante para elucidar possíveis condutas clínicas. Metodologia: Trata-se de uma revisão integrativa, que buscou responder quais os efeitos da suplementação com vitamina-D em idosos sarcopênicos, nas bases de dados: Biblioteca Virtual de Saúde (BVS), National Library of Medicine (PubMed MEDLINE) e Scientific Electronic Library Online (Scielo). Resultados e Discussão: A hipovitaminose de vitamina-D parece influenciar diretamente na fisiopatologia da sarcopenia e na função muscular. A sua suplementação parece trazer benefícios como aumento da massa e da força muscular e melhora na instabilidade postural, o que pode influenciar na diminuição do risco de quedas e na mortalidade de pacientes idosos. Contudo, um consenso ainda não foi estabelecido, devido a divergências nas variáveis estudadas.
