Risk factors for ventilator-associated pneumonia: Scope review

Resumo: The aim of the study was to identify and synthesize the literature on risk factors for Pneumonia Associated with Mechanical Ventilation in patients admitted to Intensive Care Units. This is a literature review of the Scope Review type, according to guidelines proposed by the Joanna Briggs Institute and based on the guidance of PRISMA-ScR (Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews). We identified 23 risk factors for the evolution of Pneumonia associated with mechanical ventilation. The main factors identified include: longer hospital stay, airway approach, time on invasive mechanical ventilation, severity of primary disease and prolonged use of antibiotics. As for the levels of evidence, level 3 predominated among the productions and only one study developed with an experimental model. Thus, it is concluded that it is necessary to carry out clinical trials that can elucidate, in view of the intergroup comparison approach, the different risk factors, as well as to assess the possible protective factors for the development of Pneumonia associated with Mechanical ventilation.

Esta es una revisión de la literatura del tipo Scope Review, de acuerdo con las pautas propuestas por el Instituto Joanna Briggs y basada en la guía de PRISMA-ScR (Preferred Reporting Items for Systematic Reviews and Meta-Analyzes extension for Scoping Reviews). Identificamos 23 factores de riesgo para la evolución de la neumonía asociada a la ventilación mecánica. Los principales factores identificados incluyen: estancia hospitalaria más prolongada, abordaje de la vía aérea, tiempo de ventilación mecánica invasiva, gravedad de la enfermedad primaria y uso prolongado de antibióticos. En cuanto a los niveles de evidencia, el nivel 3 predominó entre las producciones y solo un estudio se desarrolló con un modelo experimental. Así, se concluye que es necesario realizar ensayos clínicos que puedan dilucidar, a la vista del enfoque de comparación intergrupal, los diferentes factores de riesgo, así como evaluar los posibles factores protectores para el desarrollo de Neumonía asociada a la Ventilación Mecánica.O objetivo do estudo foi identificar e sintetizar a literatura sobre os fatores de risco para Pneumonia Associada à Ventilação Mecânica em pacientes internados em Unidades de Terapia Intensiva. Trata-se de uma revisão da literatura do tipo Revisão de Escopo conforme orientações propostas pelo Joanna Briggs Institute e seguindo por base o direcionamento do PRISMA-ScR (Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews). Identificou-se 23 fatores de risco para evolução de Pneumonia associada à ventilação mecânica. Os principais fatores identificados incluem: maior tempo de internação hospitalar, abordagem à via aérea, tempo em ventilação mecânica invasiva, gravidade da doença primária e uso prolongado de antibióticos. Quanto aos níveis de evidências, o nível 3, predominou dentre as produções e apenas um estudo desenvolvido com modelo experimental.
