Association between Helicobacter pylori infection and the risk of developing coronary artery disease: a scoping review

Resumo: | Helicobacter Pylori (HP) infection affects billions of people around the world, and this bacterium is considered the most prevalent infectious agent in humans. In recent years, the literature has shown a possible association between this infection and the development of coronary artery disease (CAD), since both conditions are considered chronic diseases, sharing similar pathophysiological events. The present study aims to evaluate the association between HP infection and the risk of developing CAD. For this Scoping Review, carried out through the PubMed and Cochrane Library platforms between 2022 and 2012, the chosen descriptors were ''Helicobacter Pylori'', ''Coronary Disease'' and ''Heart'', associated exclusively with the Boolean operator ''AND''. Inclusion criteria consisted of 1) clinical trials, controlled and randomized tests, reviews and meta-analyses, 2) studies on the association between CAD and PH, 3) studies that evaluated the extent of cardiovascular damage due to PH infection and 4) articles written in Portuguese, Japanese, English or Spanish. Exclusion criteria included inadequacies to the theme, case reports and studies with animals, in addition to texts outside the time limit. The evidence found suggested that the main cardiovascular adverse effects related to HP infection were associated with CAD, atherosclerosis, systemic arterial hypertension and acute myocardial infarction. Screening methods, interventional evaluations and indications for the use of laboratory markers, as well as further clinical analysis of this interaction, remain topics to be better elucidated. |

En los últimos años, la literatura ha mostrado una posible asociación entre esta infección y el desarrollo de enfermedad arterial coronaria (EAC), ya que ambas condiciones se consideran enfermedades crónicas, compartiendo eventos fisiopatológicos similares. El presente estudio tiene como objetivo evaluar la asociación entre la infección por HP y el riesgo de desarrollar EAC. Para este Scope Review, realizado a través de las plataformas PubMed y Cochrane Library entre 2022 y 2012, los descriptores elegidos fueron ''Helicobacter Pylori'', ''Coronary Disease'' y ''Heart'', asociados exclusivamente al operador booleano ' 'Y''. Los criterios de inclusión consistieron en 1) ensayos clínicos, pruebas controladas y aleatorias, revisiones y metanálisis, 2) estudios sobre la asociación entre CAD y HP, 3) estudios que evaluaron la extensión del daño cardiovascular debido a la infección por HP y 4) artículos escritos en portugués, japonés, inglés o español. Los criterios de exclusión incluyeron insuficiencias en el tema, relatos de casos y estudios con animales, además de textos fuera del límite de tiempo. La evidencia encontrada sugiere que los principales efectos adversos cardiovasculares relacionados con la infección por HP estaban asociados a EAC, aterosclerosis, hipertensión arterial sistémica e infarto agudo de miocardio. Los métodos de detección, las evaluaciones de intervención y las indicaciones para el uso de marcadores de laboratorio, así como un mayor análisis clínico de esta interacción, siguen siendo temas que deben dilucidar mejor.A infecção por Helicobacter Pylori (HP) afeta bilhões de pessoas ao redor do mundo, sendo essa bactéria considerada o agente infeccioso mais prevalente nos seres humanos. Nos últimos anos, a literatura tem evidenciado uma possível associação entre essa infecção e o desenvolvimento de doença arterial coronariana (DAC), uma vez que ambas condições são consideradas doenças crônicas, compartilhando entre si de eventos fisiopatológicos semelhantes. O presente estudo objetiva avaliar a associação entre a infecção por HP e o risco de desenvolvimento de DAC. Para essa Revisão de Escopo, realizada através das plataformas PubMed e Cochrane Library entre 2022 e 2012, os descritores eleitos foram ‘’Helicobacter Pylori’’, ‘’Coronary Disease’’ e ‘’Heart’’, associados ao operador exclusivamente ao booleano ‘’AND’’. Os critérios de inclusão consistiram 1) ensaios clínicos, testes controlados e aleatórios, revisões e meta análises, 2) trabalhos acerca da associação entre DAC e HP, 3) estudos que avaliaram a extensão dos danos cardiovasculares em decorrência da infecção por HP e 4) artigos escritos em língua portuguesa, japonesa, inglesa ou em espanhol. Os critérios de exclusão incluíram inadequações ao tema, relatos de casos e estudos com animais, além de textos fora do limite temporal. As evidências encontradas sugeriram que os principais efeitos adversos cardiovasculares relacionados à infecção por HP estavam associados à DAC, a aterosclerose, a hipertensão arterial sistêmica e ao infarto agudo do miocárdio.
