Profile of users and characteristics of deaths in a Reference Service for HIV-AIDS in Feira de Santana, Bahia, Brazil

Resumo: Introduction: Even after almost four decades of its discovery, the Acquired Immunodeficiency Virus (HIV) still has a high potential for morbidity and mortality, remaining among the main public health problems. In addition to the profile of people who are infected, the causes of death in this population have shown a trend beyond opportunistic diseases and cancer, such as chronic non-communicable diseases. Objective: To describe the profile of users and the main characteristics of deaths in a Specialized Assistance Service (SAE) of the Municipal Reference Center (CRM) for STD / HIV / AIDS in Feira de Santana, Bahia, Brazil. Materials and method: This is a sectional study, conducted based on data from a historical cohort. Sociodemographic, lifestyle, clinical complications, treatment and mortality variables obtained from these individuals' medical records were analyzed. Results: The sample studied consisted of 199 individuals who started using antiretrovirals between 2003-2017. There was a predominance of males (63.8%), self-declared blacks (82.9%), median age 34 years (18-72 years), without a partner (61.9%), heterosexuals (69.5%) and 45.7% studied up to 9 years old (elementary school). HIV-related lethality was 7%. Conclusion: The late diagnosis and the high occurrence of cases with AIDS progression also called attention, mainly to a possible failure in the management of health care. In addition, opportunistic infections, co-infections and comorbidities have been identified that appear to compromise quality of life and increase the chance of death.

Además del perfil de las personas infectadas, las causas de muerte en esta población han mostrado una tendencia más allá de las enfermedades oportunistas y el cáncer, como las enfermedades crónicas no transmisibles. Objetivo: Describir el perfil de los usuarios y las principales características de las defunciones en un Servicio de Atención Especializada (SAE) del Centro Municipal de Referencia (CRM) para ETS / VIH / SIDA en Feira de Santana, Bahía, Brasil. Materiales y método: este es un estudio seccional, realizado con base en datos de una cohorte histórica. Se analizaron variables sociodemográficas, estilo de vida, complicaciones clínicas, tratamiento y mortalidad obtenidas de la historia clínica de estos individuos. Resultados: La muestra estudiada estuvo constituida por 199 individuos que comenzaron a utilizar antirretrovirales entre 2003-2017. Predominó el sexo masculino (63,8%), negros autodeclarados (82,9%), mediana de edad 34 años (18-72 años), sin pareja (61,9%), heterosexuales (69,5%) y el 45,7% estudió hasta 9 años (primaria). La letalidad relacionada con el VIH fue del 7%. Conclusión: El diagnóstico tardío y la alta ocurrencia de casos con progresión del sida también llamaron la atención, principalmente por una posible falla en la gestión de la atención de salud. Además, se han identificado infecciones oportunistas, coinfecciones y comorbilidades que parecen comprometer la calidad de vida y aumentar la probabilidad de muerte.Introdução: Mesmo depois de quase quatro décadas da sua descoberta, o Vírus da Imunodeficiência Adquirida (HIV) ainda apresenta um elevado potencial de morbimortalidade, permanecendo entre os principais problemas de saúde pública. Além do perfil das pessoas que são infectadas, as causas de óbitos nessa população têm mostrado uma tendência para além de doenças oportunistas e câncer, a exemplo de doenças crônicas não-transmissíveis. Objetivo: Descrever o perfil dos usuários e as principais características dos óbitos num Serviço de Assistência Especializada (SAE) do Centro de Referência Municipal (CRM) para DST/HIV/Aids em Feira de Santana, Bahia, Brasil. Materiais e método: Trata-se de um estudo seccional, conduzido a partir de dados de uma coorte histórica. Foram analisadas variáveis sociodemográficas, de estilo de vida, complicações clínicas, tratamento e mortalidade obtidas dos prontuários clínicos desses indivíduos. Resultados: A amostra estudada contou com 199 indivíduos que iniciaram o uso de antirretrovirais entre os anos de 2003-2017. Houve predomínio do sexo masculino (63,8%), autodeclarados negros (82,9%), mediana de idade 34 anos (18-72 anos), sem companheiro (61,9%), heterossexuais (69,5%) e 45,7% estudaram até 9 anos (ensino fundamental). A letalidade relacionada ao HIV foi de 7%.
