Identification of multidrug-resistant strains in the hands of nursing professionals by the glove-juice method

Resumo: Although there are different methodologies for collection of microorganisms from hands, there is a lack of comparative studies. In this sense, we aimed to quantify potential pathogenic strains in the hands of nursing professionals in a public hospital in South of Brazil, based on the comparative evaluation of collection methods. For this, we carried out a cross-sectional, experimental and quali-quantitative study, during 2019, under the approval of the Ethics and Research Committee with Human Beings. We proved that glove-juice is the recommended methodology for microbiological collections from the hands, as it recovered 42.19% of the microorganisms, while the swab recovered 1.91%. From this, we carried collections, quantification and identification of microorganisms in the hands of nursing professionals. For mesophilic aerobic microorganisms, the quantification ranged from 5X104 to 4X106 CFU/hand, for coagulase positive Staphylococcus ranged from 5X104 to 3X106 CFU/hand and for fungi from 5X104 to 2X106 CFU/hand. Among the fungi, yeasts dominated in 93.73%, being frequently identified the genus Candida spp. Total and thermotolerant coliforms were not found. Staphylococcus aureus were in 90.91% of the hands, and the Antibiotic Sensitivity Test was carried out in 25 isolated colonies, all testing resistance to at least three classes of antibiotics, showing that they are multiresistant. Data analysis showed that the best collection method is glove-juice, as it recovers greater quantity and microbial diversity than the compared method. In addition, it was possible to observe the importance of hands as sources of cross-contamination between patients.

En este sentido, nos propusimos cuantificar las cepas patógenas potenciales en manos de profesionales de enfermería en un hospital público del Sur de Brasil, con base en la evaluación comparativa de los métodos de recolección. Para ello, realizamos un estudio transversal, experimental y cuali-cuantitativo, durante 2019, bajo la aprobación del Comité de Ética e Investigación con Seres Humanos. Demostramos que el enjuague de guantes es la metodología recomendada para las recolecciones microbiológicas de las manos, ya que recuperó el 42,19% de los microorganismos, mientras que el hisopo recuperó el 1,91%. A partir de esto, realizamos recolecciones, cuantificación e identificación de microorganismos en manos de profesionales de enfermería. Para los microorganismos aerobios mesófilos, la cuantificación varió de 5X104 a 4X106 UFC/mano, para Staphylococcus coagulasa positivo varió de 5X104 a 3X106 UFC/mano y para hongos de 5X104 a 2X106 UFC/mano. Entre los hongos, las levaduras predominaron en el 93,73%, siendo frecuentemente identificado el género Candida spp. No se encontraron coliformes totales y termotolerantes. Staphylococcus aureus se encontró en el 90,91% de las manos y la Prueba de Sensibilidad Antibiótica se realizó en 25 colonias aisladas, todas probando resistencia a al menos tres clases de antibióticos, demostrando que son multirresistentes. El análisis de datos mostró que el mejor método de recolección es el enjuague de guantes, ya que recupera mayor cantidad y diversidad microbiana que el método comparado. Además, se pudo observar la importancia de las manos como fuentes de contaminación cruzada entre pacientes.Apesar de existirem diferentes metodologias para coleta de microrganismos das mãos, há carência de estudos comparativos. Para os microrganismos aeróbios mesófilos a quantificação variou de 5X104 a 4X106 UFC/mão, para os Staphylococcus coagulase positiva variou de 5X104 a 3X106 UFC/mão e para os fungos de 5X104 a 2X106 UFC/mão. Dentre os fungos as leveduras dominaram em 93,73%, sendo identificado, com frequência, o gênero Candida spp. Não foram encontrados coliformes totais e termotolerantes. Staphylococcus aureus estiveram em 90,91% das mãos, sendo realizado Teste de Sensibilidade aos Antibióticos em 25 colônias isoladas, todas testando resistência a pelo menos três classes de antibióticos, mostrando serem multirresistentes. A análise dos dados mostrou que o melhor método de coleta é o de enxágue com luva, pois recupera maior quantidade e diversidade microbiana que o método comparado.
