Cardiac dose in radiotherapy treatments for left-sided breast cancer: use of the Deep Inspiration Breath Hold technique

Resumo: Introduction: radiotherapy treatment for left-sided breast cancer has radiation-induced cardiotoxicity as a consequence. The Deep Inspiration Breath Hold (DIBH) technique arose from the need to minimize cardiac doses. Objective: to describe the doses to cardiac structures by means of the Dose Volume Histogram (DVH) in radiotherapy treatment for left-sided breast cancer with the DIBH technique. Methodology: the research is characterized as a case study with multiple analysis units, typified as descriptive, retrospective in nature and with a quantitative approach. Results: the sample was composed of five analysis units. The average cardiac dose for the DIBH technique was 6 Gy; and for the left anterior descending coronary artery, it was 14.8 Gy. Four analysis units benefited the most from the technique, the result of which is justified by the conservative surgical modality, while one unit showed the least benefit, since it performed mastectomy.  Conclusion: DIBH is an alternative to reduce cardiac toxicity in women undergoing radiation therapy for left-sided breast cancer, especially in individuals with other risk factors associated with heart disease.

En función de la necesidad de minimizar las dosis cardíacas, surgió la técnica Deep Inspiration Breath Hold (DIBH). Objetivo: describir las dosis en las estructuras cardíacas mediante el Histograma Dosis-Volumen (DVH) en el tratamiento con radioterapia del cáncer en la mama izquierda con la técnica DIBH. Metodología: la investigación se caracteriza por ser un estudio de caso con múltiples unidades de análisis, del tipo descriptivo, de naturaleza retrospectiva y con un enfoque cuantitativo. Resultados: la muestra estaba compuesta por cinco unidades de análisis. La dosis cardíaca promedio para la técnica DIBH fue de 6 Gy; y para la arteria coronaria descendente anterior, esta fue de 14,8 Gy. Cuatro unidades de análisis fueron las que más se beneficiaron de la técnica, cuyo resultado se justifica por la modalidad quirúrgica conservadora, mientras que una unidad presentó el menor beneficio, ya que realizó la mastectomía.  Conclusión: el DIBH es una alternativa para reducir la toxicidad cardíaca en las mujeres que se someten a la radioterapia para el cáncer en la mama izquierda, especialmente en individuos con otros factores de riesgo asociados a la enfermedad cardíaca.Introdução: o tratamento radioterápico para câncer de mama esquerda tem como consequência a cardiotoxicidade induzida por radiação. A partir da necessidade de minimizar as doses cardíacas, surgiu a técnica Deep Inspiration Breath Hold (DIBH). Objetivo: descrever as doses nas estruturas cardíacas por meio do Histograma Dose Volume (DVH) no tratamento radioterápico para câncer de mama esquerda com a técnica DIBH. Metodologia: a pesquisa se caracterizada como estudo de caso com múltiplas unidades de análise, do tipo descritivo, de natureza retrospectiva e com abordagem quantitativa. Resultados: a amostra foi composta por cinco unidades de análise. A dose média cardíaca para a técnica DIBH foi de 6 Gy; e para a artéria coronária descendente anterior esquerda, esta foi de 14,8 Gy.
