Burnout in oral and maxillofacial surgeons in Brazilian population during the COVID-19 pandemic: a critical analysis

Resumo: | The training and activities of an oral and maxillofacial surgeon, although stimulating, motivating, and satisfactory, are not exempt from technical and psychological challenges. Residency training programs aim to develop necessary skills of resistance and rational decision-making under pressure in clinical, outpatient, and complex hospital interventions. Furthermore, these programs also require the fulfillment of activities outside the operating room, such as issuing diagnoses and planning high complexity procedures with a high risk of complications. The aim of this study was to evaluate the levels of burnout in attending surgeons and residents from oral and to maxillofacial surgery residency programs located in the State of Ceará, Brazil, during the SARS CoV-2 (COVID-19) disease pandemic. This observational, cross-sectional study with a quantitative approach was conducted on attending surgeons and residents registered at the Regional Council of Dentistry – Ceará Section. Data were obtained through an online questionnaire with three sets of questions: 1) professional factors; 2) the Oldenburg Burnout Inventory instrument; and 3) sociodemographic aspects. Quantitative data were analyzed with the Statistical Package for Social Science software. Nonparametric statistical analysis (Fisher and Kruskal-Wallis Exact tests) was used, in addition to the Chi-square test, adopting a significance level of 5%. A sample of 78 participants was obtained, in which most attendings (58.7%) and residents (40%) demonstrated moderate levels of burnout. We can conclude that the levels of burnout in oral and maxillofacial attending surgeons and residents from the State of Ceará during the COVID-19 pandemic were moderate, not resulting in high levels of emotional exhaustion and depersonalization. |

Los programas de formación en residencia tienen como objetivo desarrollar las habilidades de resistencia necesarias y la toma de decisiones racionales bajo presión en intervenciones clínicas, ambulatorias y hospitalarias complejas. Además, estos programas también requieren la realización de actividades fuera del quirófano, como la emisión de diagnósticos y la planificación de procedimientos de alta complejidad y alto riesgo de complicaciones. El objetivo de este estudio fue evaluar los niveles de burnout en cirujanos y residentes de programas de residencia en cirugía oral y maxilofacial ubicados en el estado de Ceará, Brasil, durante la pandemia de la enfermedad del SARS CoV-2 (COVID-19). Este estudio observacional, transversal, con abordaje cuantitativo, se realizó con cirujanos asistentes y residentes registrados en el Conselho Regional de Odontologia - Seção Ceará. Los datos se obtuvieron a través de un cuestionario en línea con tres conjuntos de preguntas: 1) factores profesionales; 2) el instrumento Oldenburg Burnout Inventory; y 3) aspectos sociodemográficos. Los datos cuantitativos se analizaron con el software Statistical Package for Social Science. Se utilizó análisis estadístico no paramétrico (pruebas exactas de Fisher y Kruskal-Wallis), además de la prueba de chi-cuadrado, adoptando un nivel de significancia del 5%. Se obtuvo una muestra de 78 participantes, en la que la mayoría de los pacientes (58,7%) y residentes (40%) presentaban un nivel moderado de burnout. Podemos concluir que los niveles de burnout en los cirujanos asistentes y residentes del Estado de Ceará durante la pandemia COVID-19 fueron moderados, no resultando en altos niveles de agotamiento emocional y despersonalización.A formação e as atividades do cirurgião bucomaxilofacial, embora estimulantes, motivadoras e satisfatórias, não estão isentas de desafios técnicos e psicológicos. Os programas de treinamento em residência visam desenvolver as habilidades necessárias de resistência e tomada de decisão racional sob pressão em intervenções clínicas, ambulatoriais e hospitalares complexas. Além disso, esses programas também exigem a realização de atividades fora da sala de cirurgia, como a emissão de diagnósticos e o planejamento de procedimentos de alta complexidade e com alto risco de complicações. O objetivo deste estudo foi avaliar os níveis de burnout em cirurgiões e residentes de programas de residência em cirurgia oral e maxilofacial localizados no Estado do Ceará, Brasil, durante a pandemia da doença SARS CoV-2 (COVID-19). Este estudo observacional, transversal, com abordagem quantitativa, foi realizado com cirurgiões atendentes e residentes cadastrados no Conselho Regional de Odontologia - Seção Ceará. Os dados foram obtidos por meio de um questionário online com três conjuntos de perguntas: 1) fatores profissionais; 2) o instrumento Oldenburg Burnout Inventory; e 3) aspectos sociodemográficos. Os dados quantitativos foram analisados com o software Statistical Package for Social Science. Foi utilizada análise estatística não paramétrica (testes Exatos de Fisher e Kruskal-Wallis), além do teste Qui-quadrado, adotando-se nível de significância de 5%. Obteve-se uma amostra de 78 participantes, em que a maioria dos atendimentos (58,7%) e residentes (40%) apresentou nível moderado de burnout.
