Risk behaviors after kidney transplant that influence the adherence to treatment

Resumo: The objectives of the study were to identify the main risk behaviors of post-transplant chronic kidney patients and to relate these behaviors to the outcome of non-adherence. Method: This was an observational, descriptive, retrospective, cross-sectional study with a quantitative approach, whose sample consisted of medical records of post-kidney transplant patients who had at least one nursing consultation at the post-transplant clinic in a university hospital located in the state of Rio de Janeiro, from August 2016 to December 2020. The analysis was performed from the data collected, using the Statistical Package for Social Sciences - (SPSS), version 24.0, for Windows. Results: Ninety-eight medical records were evaluated, and significant rates were identified in the following behaviors: failure to follow the diet (30.5%), delay or forgetfulness in the use of medications (17.9%), alcoholism (8.4%), hygiene inadequate (1.1%), smoking (1.1%), recurrent urinary tract infection (1.1%), irregular use of sunscreen (1.1%), increased creatinine (1.1%) and sedentary lifestyle (1.1%). Conclusion: In this study, it was possible to identify problems in the established therapy and, consequently, the presence of risk behaviors after kidney transplantation, which may influence the survival of the transplanted graft. Nursing can and should act to strengthen the self-care of these patients aiming at the possibility of a better therapeutic response.

Método: Este fue un estudio observacional, descriptivo, retrospectivo, transversal con enfoque cuantitativo, cuya muestra estuvo constituida por historias clínicas de pacientes pos trasplante renal que tuvieron al menos una consulta de enfermería en el ambulatorio pos trasplante de una hospital de universidad en el estado de Río de Janeiro, de agosto de 2016 a diciembre de 2020. El análisis se realizó a partir de los datos recolectados, utilizando el Paquete Estadístico para Ciencias Sociales - (SPSS), versión 24.0, para Windows. Resultados: Se evaluaron 98 prontuarios y se identificaron tasas significativas en las siguientes conductas: incumplimiento de la dieta (30,5%), retraso u olvido en el uso de medicamentos (17,9%), alcoholismo (8,4%), higiene inadecuado (1,1%), tabaquismo (1,1%), infección urinaria recurrente (1,1%), uso irregular de protector solar (1,1%), aumento de creatinina (1,1%) y sedentarismo (1,1%). Conclusión: En este estudio, fue posible identificar problemas en la terapia establecida y, consecuentemente, la presencia de comportamientos de riesgo después del trasplante renal, que pueden influir en la supervivencia del injerto trasplantado. La enfermería puede y debe actuar para fortalecer el autocuidado de estos pacientes buscando la posibilidad de una mejor respuesta terapéutica.Os objetivos do estudo foram identificar os principais comportamentos de risco dos pacientes renais crônicos pós-transplante e relacionar tais comportamentos com o desfecho da não adesão. Método: Tratou-se, de um estudo observacional, descritivo, retrospectivo, de corte transversal e com abordagem quantitativa, cuja amostra foi constituída de prontuários de pacientes pós- transplante renal, que realizaram ao menos uma consulta de enfermagem no ambulatório de pós-transplante renal de um hospital universitário localizado no estado do Rio de Janeiro, no período de agosto de 2016 a dezembro de 2020. A análise foi realizada a partir dos dados coletados, utilizando o programa Statistical Package for Social Sciences – (SPSS), versão 24.0, para Windows. Resultados: Foram avaliados 98 prontuários, sendo identificadas taxas expressivas nos seguintes comportamentos: falha ao seguir a dieta (30,5%), atraso ou esquecimento no uso das medicações (17,9%), etilismo (8,4%), higiene inadequada (1,1%), tabagismo (1,1%), infecção urinária de recorrência (1,1%), uso irregular de protetor solar (1,1%), aumento de creatinina (1,1%) e sedentarismo (1,1%). Conclusão: Neste estudo, foi possível identificar problemas na terapêutica instituída e por consequência a presença de comportamentos de risco pós-transplante renal, que podem vir a influenciar a sobrevida do enxerto transplantado.
