Efficiency in the management of public health resources in the municipalities of the Northeast: A two-step approach

Resumo: | The problems faced by public managers in the health area range from the limitation of public resources to the high demand of society for the services provided by the Unified Health System (SUS), which recommends that these managers value efficiency in the use of resources. In this context, this study seeks to measure the technical efficiency scores and the scale of health expenditures in 1694 municipalities in the Northeast and to identify the determinants of this efficiency. For this, the Data Envelopment Analysis (DEA) model and the quantile regression model were used, using secondary data having 2019 as the base year. The main results show that few municipalities are at a fully efficient level and that most of them are in the class between 0.50 and 0.75 efficiency level. From the quantile regression, it is noted that there are socioeconomic and demographic factors that are associated with different levels of efficiency. A relevant fact is that the municipality that spent the most on health per inhabitant had the lowest efficiency score, in the DEA model with constant returns to scale. This indicates that higher expenditures are not necessarily associated with higher efficiency scores. |

En ese contexto, este estudio busca medir los puntajes de eficiencia técnica y la escala de los gastos en salud en 1694 municipios del Nordeste e identificar los determinantes de esa eficiencia. Para ello se utilizó el modelo de Análisis Envolvente de Datos (DEA) y el modelo de regresión por cuantiles, utilizando datos secundarios teniendo como año base el 2019. Los principales resultados muestran que pocos municipios se encuentran en un nivel de eficiencia plena y que la mayoría de ellos se encuentran en el clase entre 0,50 y 0,75 nivel de eficiencia. De la regresión por cuantiles se observa que existen factores socioeconómicos y demográficos que se asocian a diferentes niveles de eficiencia. Un dato relevante es que el municipio que más gastó en salud por habitante obtuvo el puntaje de eficiencia más bajo, en el modelo DEA con rendimientos constantes a escala. Esto indica que los gastos más altos no están necesariamente asociados con puntajes de eficiencia más altos.Os problemas enfrentados pelos gestores públicos na área da saúde, vão desde a limitação dos recursos públicos a alta demanda da sociedade pelos serviços disponibilizados pelo Sistema Único de Saúde (SUS), o que preconiza que esses gestores prezem pela eficiência na utilização dos recursos. Nesse contexto, este estudo busca mensurar os escores de eficiência técnica e de escala dos gastos com saúde de 1694 municípios do Nordeste e identificar os fatores determinantes dessa eficiência. Para tanto, empregou-se o modelo de Análise Envoltória de Dados (DEA) e o modelo de regressão quantílica, utilizando dados secundários tendo como ano base 2019. Os principais resultados mostram que poucos municípios estão em um nível totalmente eficiente e que a maioria deles estão na classe entre 0,50 e 0,75 de nível de eficiência. A partir da regressão quantílica nota-se que existem fatores socioeconômicos e demográficos que estão associados aos diferentes níveis de eficiência. Um fato relevante é que o município que mais gastou com saúde por habitante foi o que teve o menor escore de eficiência, no modelo DEA com retornos constantes à escala.
