Inferior natal teeth detected within the first hours of life: report case

Resumo: | The objective of this article is to report a clinical case of a patient with a natal tooth presenting the clinical characteristics, causes and treatment. The case report shows a female patient, new born with 30 hours of life, presenting respiratory distress and severe hypothermia (31.3ºC) after delivery, soon resolved by the responsible medical team. In the first hours of life, the presence of two lower teeth in the dental arch was detected and pointed out for making breast feeding difficult, been referred to the Dentistry service of the University Hospital Lauro Wanderley. Intraoral physical examination showed no lesions or trauma to the mucosa, and the presence of two teeth in the region of the lower central incisors that showed accentuated mobility was verified. Due to the difficulty in breastfeeding, tooth mobility and concern about teeth being aspirated, it became necessary to remove the elements. The procedure was performed under topical local anesthesia with 5% Lidocaine spray, applied on a cotton swab. Then, a Molt detacher was used, removing part of the mucosa that covered the teeth and the excision of the two dental elements was performed. Suture was not necessary and there were no complications during the surgical procedure. It is concluded that this rare condition requires multidisciplinary work between the team responsible for the pregnant woman and the new born, who must recognize this anomaly as soon as possible in order to be able to refer to a dentist to define together what is the best conduct. |

El caso clínico mostra una paciente femenino, recién nacido con 30 horas de vida, presentando dificultad respiratoria e hipotermia severa (31.3ºC) después del parto, pronto resuelto por el equipo médico. Incluso en las primeras horas de vida, se detectó la presencia de dientes inferiores en el arco dental y se señaló que dificultaba la lactancia, después de haber sido derivada al servicio de Odontología del Hospital Universitario Lauro Wanderley. El examen intraoral no mostró lesiones ni traumas en la mucosa y se verificó la presencia de dientes en la región de los incisivos centrales inferiores que presentaban una movilidad marcada. Debido a la dificultad de la lactancia, la movilidad de los dientes y las preocupaciones sobre la aspiración de los dientes, se hizo necesario retirar los elementos. El procedimiento se realizó bajo anestesia tópica con spray de lidocaína al 5%, aplicado sobre un bastoncillo de algodón. Luego, se usó um elevador perióstico de Molt, quitando parte de la mucosa que cubría los dientes y realizando la escisión de los dos elementos. No fue necesario realizar sutura y no hubo complicaciones durante el procedimiento. Se concluye que esta condición requiere un trabajo multidisciplinario entre el equipo responsable de la mujer embarazada y el recién nacido, que debe reconocer esta anomalía lo antes posible para poder referirse a un dentista para definir juntos cuál es lamejorconducta.O objetivo desse artigo é relatar um caso clínico de paciente com dente natal apresentando as características clínicas, causas e tratamento. Relata-se o caso de uma paciente do sexo feminino, recém-nascido com 30 horas de vida, apresentando após o parto desconforto respiratório e hipotermia grave (31,3ºC) brevemente solucionados pela equipe médica responsável. Ainda nas primeiras horas de vida, foi detectado presença de dois dentes inferiores na arcada dentária e apontados por dificultarem o aleitamento materno, tendo sido encaminhado para o serviço de Odontologia, do Hospital Universitário Lauro Wanderley.Ao exame físico intraoral não foi visto lesões ou traumas em mucosa e comprovou-se a presença de dois dentes na região dos incisivos centrais inferiores que apresentavam mobilidade acentuada. Em virtude da dificuldade de aleitamento, mobilidade dentária e preocupação dos dentes serem aspirados, tornou-se necessário fazer a remoção dos elementos. O procedimento foi realizado sob anestesia local tópica com Lidocaína spray 5%, aplicada em haste de algodão. Em seguida, foi utilizado um descolador de Molt, remoção de fina película de mucosa que recobria os dentes e realizado a exérese dos dois elementos dentários.
