Surgical management of natal and neonatal teeth in patients with cleft lip and palate – Case Report

Resumo: Background: Natal teeth are defined as teeth present at birth, while neonatal dental elements are those that hatch within 30 days after this event, and these changes from normality are more frequent in babies with cleft lip and palate. Conservative treatment consists of follow-up in cases where there is no harm to the mother and child. However, the presence of these elements can cause discomfort during suction, laceration of the mother's breasts and aspiration of teeth, requiring surgical removal. Objective and case report: the aim of this work is to present two cases of natal and neonatal teeth in babies with cleft lip and palate. The first case is intended to report a neonatal tooth in the anterior region of the mandible. The second, in sequence, reports the presence of a natal tooth in the maxillary region, both of which are deciduous dentition. The treatment of choice for the two cases mentioned above involved the removal of the elements and subsequent follow-up, a therapy chosen due to suction difficulties, mobility that can predispose to accidental aspiration of the element by the baby, injuries to the breast during breastfeeding and damage to molding in cases where there is a need to make intraoral plates to help with feeding. Conclusion: Although conservative treatment is a viable option in some cases of natal and neonatal teeth in patients with cleft lip and palate, surgical removal is an effective therapy in cases where the dental element causes significant damage to the mother and baby.

El tratamiento conservador consiste en un seguimiento en los casos en los que no haya daño a la madre y al niño. Sin embargo, la presencia de estos elementos puede ocasionar molestias durante la succión, laceración de los senos de la madre y aspiración de dientes, requiriendo extirpación quirúrgica. Objetivo y caso clínico: el objetivo de este trabajo es presentar dos casos de dientes natales y neonatales en bebés con labio leporino y paladar hendido. El primer caso está destinado a informar de un diente neonatal en la región anterior de la mandíbula. El segundo, en secuencia, informa la presencia de un diente natal en la región maxilar, ambos de los cuales son dentición decidua. El tratamiento de elección para los dos casos mencionados anteriormente implicó la remoción de los elementos y posterior seguimiento, una terapia elegida por dificultades de succión, movilidad que puede predisponer a la aspiración accidental del elemento por parte del bebé, lesiones en la mama durante la lactancia. y daño a la moldura en los casos en que es necesario hacer placas intraorales para ayudar con la alimentación. Conclusión: Aunque el tratamiento conservador es una opción viable en algunos casos de dientes natales y neonatales en pacientes con labio leporino y paladar hendido, la extirpación quirúrgica es una terapia eficaz en los casos en que el elemento dental causa un daño significativo a la madre y al bebé.Fundamento: Dentes natais são definidos como dentes presentes ao nascimento, enquanto elementos dentários neonatais são os que eclodem em até 30 dias após esse evento, sendo estas alterações da normalidade mais frequentes em bebês portadores de fissuras labiopalatinas. O tratamento conservador consiste em acompanhamento nos casos em que não há prejuízo à mãe e à criança. No entanto, a presença desses elementos pode causar desconforto durante a sucção, laceração dos seios da mãe e aspiração dos dentes, necessitando de remoção cirúrgica. Objetivo e relato de caso: o objetivo do trabalho é apresentar dois casos de dentes natal e neonatal em bebês com fissura labiopalatina. O primeiro caso destina-se ao relato de um dente neonatal na região anterior de mandíbula. O segundo, em sequência, relata a presença de dente natal em região maxilar, sendo ambas dentições decíduas. O tratamento de escolha para os dois casos supracitados envolveu a remoção dos elementos e posterior acompanhamento, terapêutica escolhida em decorrência das dificuldades de sucção, mobilidade que pode predispor à aspiração acidental do elemento pelo bebê, ferimentos ao seio materno durante a amamentação e prejuízo em moldagens nos casos em que há a necessidade da confecção de placas intraorais para auxiliar na alimentação.
