Protective effects of influenza vaccine in patients with coronary artery disease: a systematic review

Resumo: Objective: To analyze the main protective effects of the influenza vaccine in patients with coronary artery disease. Methodology: This is a systematic review carried out using the PRISMA method, in the electronic databases PubMed, Scopus and Web of Science, using the following descriptors: “influenza vaccines”, “influenza”, “flu”, “coronary artery disease”, “myocardial infarction” and “acute coronary syndrome”. Full articles in Portuguese, English or Spanish published between January 2006 and December 2021 were included; studies that evaluated the effect of influenza vaccine in adult patients (≥18 years of age) with coronary artery disease; studies that included placebo or no vaccine as a control group; studies that reported at least one evaluated outcome and with statistical association. Results: 04 articles composed the final sample of this study. Regarding the approach of the articles found, it was found that three studies used the Randomized Clinical Trial, characterizing the studies as presenting a strong level of evidence (II). In all studies analyzed, influenza vaccine significantly reduced the risk of MACE (cardiovascular death, myocardial infarction, coronary revascularization) compared with the control group. Furthermore, in these studies the influenza vaccine reduced cardiovascular mortality and the risk of acute myocardial infarction. Conclusion: It was found that the influenza vaccine can significantly reduce cardiovascular events in patients with coronary artery disease.

Metodología: Se trata de una revisión sistemática realizada mediante el método PRISMA, en las bases de datos electrónicas PubMed, Scopus y Web of Science, utilizando los siguientes descriptores: “influenza vaccines”, “influenza”, “flu”, “coronary artery disease”, “myocardial infarction” y “acute coronary syndrome”. Se incluyeron artículos completos en portugués, inglés o español publicados entre enero de 2006 y diciembre de 2021; estudios que evaluaron el efecto de la vacuna contra la influenza en pacientes adultos (≥18 años) con enfermedad arterial coronaria; estudios que incluyeron placebo o ninguna vacuna como grupo de control; estudios que reportaron al menos un desenlace evaluado y con asociación estadística. Resultados: 04 artículos compusieron la muestra final de este estudio. En cuanto al abordaje de los artículos encontrados, se constató que tres estudios utilizaron el Ensayo Clínico Aleatorizado, caracterizándose los estudios como de fuerte nivel de evidencia (II). En todos los estudios analizados, la vacuna contra la influenza redujo significativamente el riesgo de MACE (muerte cardiovascular, infarto de miocardio, revascularización coronaria) en comparación con el grupo control. Además, en estos estudios la vacuna antigripal redujo la mortalidad cardiovascular y el riesgo de infarto agudo de miocardio. Conclusión: Se encontró que la vacuna contra la influenza puede reducir significativamente los eventos cardiovasculares en pacientes con enfermedad de la arteria coronaria.Objetivo: Analisar os principais efeitos protetores da vacina contra influenza em pacientes com doença arterial coronariana. Foram incluídos artigos completos em português, inglês ou espanhol, publicados entre janeiro de 2006 a dezembro de 2021; estudos que avaliaram o efeito da vacina contra influenza em pacientes adultos (≥18 anos de idade) com doença arterial coronariana; estudos que incluíram placebo ou nenhuma vacina como grupo controle; estudos que relataram pelo menos um resultado avaliado e com associação estatística. Resultados: 04 artigos compuseram a amostra final deste estudo. Referente à abordagem dos artigos encontrados, verificou-se que três pesquisas utilizaram o Ensaio Clínico Randomizado, caracterizando as pesquisas como apresentando nível de evidência forte (II). Em todos os estudos analisados, a vacina contra influenza reduziu significativamente o risco de MACE (morte cardiovascular, infarto do miocárdio, revascularização coronariana) em comparação com o grupo controle. Além disso, nestes estudos a vacina contra influenza reduziu a mortalidade cardiovascular e o risco de infarto agudo do miocárdio.
