Patterns of defects of post-critical patients for quality control performance evaluation

Resumo: This study aims to organize the observable characteristics of post-critical patients in order to form standards of conformity or non-compliance to be used in performance evaluations in the quality control of health care production in intensive care units. Having hypothesized that the existence of different observable characteristics of post-critical patients determines patterns of intensity of patient defects, due to the progression of disease deterioration, even after critical care, which will require the development of different nursing services in the long term. This is a descriptive-exploratory study, with a qualitative approach, carried out in three phases: literature review, construction of attributes and their sublevels and organization of attributes and their levels. From the scientific literature, the classification of the observable characteristics of post-critical patients was elaborated, weighted by the intensity of the "defect" of clinical deterioration. Soon after, the tree diagram of the clinical dimension used to organize the attributes and their levels was elaborated, and later, an attribute was extracted as an example to illustrate the application of the tree diagram in practice. Therefore, 9 attributes were elaborated with stratification from 3 to 8 levels, totaling 39 levels. It was evidenced that it is possible to group the observable characteristics of post-critical patients by different intensities of patterns of defects due to the progression of disease deterioration, even after patients have undergone critical care, which will require the development of different nursing services in the long term.

Habiendo hipotetizado que la existencia de diferentes características observables de pacientes post-críticos determina patrones de intensidad de defectos del paciente, debido a la progresión del deterioro de la enfermedad, incluso después de cuidados críticos, que requerirán el desarrollo de diferentes servicios de enfermería a largo plazo. Se trata de un estudio descriptivo-exploratorio, con un enfoque cualitativo, realizado en tres fases: revisión de la literatura, construcción de atributos y sus subniveles y organización de atributos y sus niveles. A partir de la literatura científica, se elaboró la clasificación de las características observables de los pacientes post-críticos, ponderado por la intensidad del "defecto" del deterioro clínico. Poco después, se elaboró el diagrama de árbol de la dimensión clínica utilizada para organizar los atributos y sus niveles, y más tarde, se extrajo un atributo como ejemplo para ilustrar la aplicación del diagrama de árbol en la práctica. Por lo tanto, se elaboraron 9 atributos con estratificación de 3 a 8 niveles, con un total de 39 niveles. Se evidencó que es posible agrupar las características observables de los pacientes post-críticos por diferentes intensidades de patrones de defectos debido a la progresión del deterioro de la enfermedad, incluso después de que los pacientes se hayan sometido a cuidados críticos, lo que requerirá el desarrollo de diferentes servicios de enfermería a largo plazo.Este estudo tem como objetivo organizar as características observáveis dos doentes pós-críticos com intuito de formar padrões de conformidade ou não conformidade para ser usados nas avaliações de desempenho no controle de qualidade da produção de assistência à saúde nas unidades de terapia intensiva. Tendo como hipótese que a existência de diferentes características observáveis dos doentes pós-críticos determina padrões de intensidade de defeitos de doentes, pela progressão da deterioração da doença, mesmo após os cuidados críticos, que vão demandar a elaboração de diferentes serviços de enfermagem em longo prazo. Trata-se de um estudo do tipo descritivo-exploratório, de abordagem qualitativa, realizado em três fases: revisão de literatura, construção dos atributos e seus subníveis e organização dos atributos e seus níveis. Da literatura científica foi elaborada a classificação das características observáveis dos doentes pós-críticos, ponderada pela intensidade do “defeito” da deterioração clínica. Logo em seguida, foi elaborado o diagrama em árvore da dimensão clínica usado para organizar os atributos e seus níveis, e, posteriormente, extraiu-se um atributo como exemplo para ilustrar a aplicação do diagrama de árvore na prática. Diante disso, foram elaborados 9 atributos com estratificação de 3 a 8 níveis, perfazendo um total de 39 níveis.
