Environmental predictors to falls in the elderly: an integrative review

Resumo: | Objective: To identify in the literature the environmental predictors of falls in the elderly. Method: an integrative literature review included investigations developed that addressed falls in the environment of the elderly without a time frame. Revisions, studies without specific analyzes for falls in the environment, and studies with the elderly population in special situations were excluded. The Lilacs, Scopus, CINAHL, Web of Science and PubMed databases were consulted, using the descriptor accidents by falls, their correspondents in Portuguese and the Boolean operator AND. To assess the quality of the studies, the Newcastle-Ottawa scale was used. After applying the inclusion and exclusion criteria, 12 articles were selected. Results: Seven studies were found: three cross-sectional studies and four cohorts. The sample sizes ranged from 18 to 1,243 elderly people and year of publication from 1998 to 2017, the most recent of which were 2013, 2015 and 2017. The studies addressed balance and gait, self-reported falls and the prevalence of environmental risks in their majority. Conclusion: It was evidenced that the individual and environmental multifactors predispose elderly to falls. |

Método: una revisión integral de la literatura incluyó investigaciones desarrolladas que abordaron las caídas en el entorno de los ancianos sin un marco de tiempo. Se excluyeron las revisiones, los estudios sin análisis específicos para caídas en el medio ambiente y los estudios con la población de edad avanzada en situaciones especiales. Se consultaron las bases de datos Lilacs, Scopus, CINAHL, Web of Science y PubMed, utilizando el descriptor de accidentes por caídas, sus corresponsales en portugués y el operador booleano AND. Para evaluar la calidad de los estudios, se utilizó la escala Newcastle-Ottawa. Después de aplicar los criterios de inclusión y exclusión, se seleccionaron 12 artículos. Resultados: se encontraron siete estudios: tres estudios transversales y cuatro cohortes. Solo se realizó un estudio en Brasil, todos los demás publicados en inglés. El tamaño de la muestra varió de 18 a 1.243 personas mayores y año de publicación de 1998 a 2017, el más reciente de los cuales fue 2013, 2015 y 2017. Los estudios abordaron el equilibrio y la marcha, las caídas autoinformadas y la prevalencia de riesgos ambientales en su mayoría. Conclusión: se evidenció que los multifactores individuales y ambientales predisponen a las personas mayores a las caídas y al lugar donde hay una mayor incidencia de caídas y al baño seguido de la cocina.Objetivo: Identificar na literatura os preditores ambientais de quedas em idosos. Metodologia: revisão integrativa da literatura incluiu-se investigações desenvolvidas que abordassem quedas no ambiente do idosos sem recorte temporal. Excluiu-se as revisões, estudos sem análises especificas para quedas no ambiente, e estudos com população de idosos em situações especiais. Consultou-se as bases de dados Lilacs, Scopus, CINAHL, Web of Science e portal PubMed, utilizando o descritor acidentes por quedas, seus correspondentes em português e o operador boleano AND. Para avaliar a qualidade dos estudos utilizou a escala de Newcastle-Ottawa. Após a aplicação dos critérios de inclusão e exclusão, foram selecionados 12 artigos. Resultados: Localizaram-se 07 estudos sendo: três estudos transversais e quatro coortes. O tamanho das amostras variou de 18 a 1.243 idosos e ano de publicação de 1998 a 2017, sendo os mais recentes 2013, 2015 e 2017. Os estudos abordaram equilíbrio e marcha, quedas autorreferidas e prevalência de riscos ambientais em sua maioria.
