Weight change in patients under treatment with antidepressants: A narrative review

Resumo: Antidepressants are used in the treatment of several pathologies with a high incidence and prevalence. However, they are associated with several side effects, such as weight gain or loss. Thus, the objective of this study was to elaborate a narrative review about the change in body weight caused by antidepressants, as well as the possible biopsychosocial consequences of this. For this purpose, scientific articles, books and bulletins from the Ministry of Health were consulted. Studies indicated that antidepressants differ in terms of the possibility of causing weight change, in addition to the existence of multiple factors which may interfere in the expression of these effects. Furthermore, not all antidepressants of the same class promote weight change following the same pattern. Thus, in the initial treatment period, they can cause weight gain, loss or no change at all. However, in the long term, the majority seem to cause weight gain, including certain selective serotonin reuptake inhibitors, a fact unknown to some people who use them for weight loss. Bupropion was the only antidepressant shown to significantly contribute to weight loss, even during long-term treatment. It was also found that weight gain might influence treatment abandonment, which in turn can lead to relapses and/or worsening of the underlying pathology. In contrast, indiscriminate use aimed at weight loss can interfere with mood, well-being, and behaviour. In view of the aforementioned, it is important that physicians have up-to-date knowledge on the subject, in order to properly prescribe medications and correctly advise their patients.

Sin embargo, están asociados con varios efectos secundarios, como el aumento o la pérdida de peso. Así, el objetivo de este estudio fue elaborar una revisión narrativa sobre la alteración del peso provocada por los antidepresivos y abordar las posibles consecuencias biopsicosociales de este. Para ello, se consultaron artículos científicos, libros y boletines del Ministerio de Salud. Los estudios indican que los antidepresivos difieren en cuanto a la posibilidad de provocar cambios de peso, además de la existencia de múltiples factores que pueden interferir en la expresión de estos efectos. Adicionalmente, no todos los antidepresivos de la misma clase promueven el cambio de peso siguiendo el mismo patrón. Por lo tanto, inicialmente, pueden causar aumento o pérdida de peso o ningún cambio en absoluto. Sin embargo, en los tratamientos a largo plazo, la mayoría parece causar aumento de peso, incluidos algunos inhibidores selectivos de la recaptación de serotonina, un hecho desconocido para las personas que los usan para perder peso. El antidepresivo bupropión es el único que parece contribuir significativamente a la pérdida de peso, incluso en tratamientos a largo plazo. Se encontró también que el aumento de peso puede influir en el abandono del tratamiento, lo que en este caso puede conducir a recaídas y/o empeoramiento de la patología de base. Por el contrario, el uso indiscriminado destinado a la pérdida de peso puede interferir en el estado de ánimo, bienestar y comportamiento. Ante esto, es importante que los médicos estén actualizados en el tema, para prescribir adecuadamente los medicamentos y asesorar correctamente a sus pacientes.Os antidepressivos são usados no tratamento de diversas patologias de elevada incidência e prevalência. Entretanto, estão associados a diversos efeitos colaterais, como ganho ou perda de peso. Assim, o objetivo deste trabalho foi elaborar uma revisão narrativa de literatura sobre a alteração de peso causada por antidepressivos, além de abordar as possíveis consequências biopsicossociais disso. Para tal, foram consultados artigos científicos, livros e boletins Ministério da Saúde. Os estudos indicam que os antidepressivos se diferenciam quanto à possibilidade de causarem alteração de peso, além de existirem múltiplos fatores que podem interferir na expressão desses efeitos. Ademais, nem todos os antidepressivos de uma mesma classe promovem alteração de peso seguindo um mesmo padrão. Assim, inicialmente, podem causar ganho, perda ou nenhuma alteração de peso. Entretanto, em tratamentos de longo prazo, a maioria parece causar aumento de peso, incluindo alguns inibidores seletivos da recaptação de serotonina, fato esse, desconhecido por parte das pessoas que os usam objetivando perda de peso. O antidepressivo bupropiona é o único que parece contribuir significativamente para a perda de peso, inclusive em tratamento de longo prazo. Verificou-se também que o aumento de peso pode influenciar no abandono do tratamento, o que nesse caso, pode gerar recaídas e/ou piora da patologia de base. Em contrapartida, o uso indiscriminado visando a perda de peso pode interferir com o humor, bem-estar e comportamento.
