Dental management of patients with Chronic Kidney Disease: A literature review

Resumo: | Chronic kidney disease (CKD) is a public health problem that has been highlighted in recent years. It is a disease caused by the destruction of the renal functional unit, called the nephron, and which has different stages until it leads to total kidney failure. This work aims to elucidate the particularities related to dental care for people with chronic kidney disease, on hemodialysis and kidney transplant recipients, in addition to elucidating protocols on care in caring for these patients in the routine of the dental office. To carry out this narrative review, the Scielo and Pubmed databases were used, with the following keywords: “chronic renal failure” “dentistry” and “oral manifestations” associated by the Boolean operator “E”, as well as their English counterparts . Articles were selected without limiting the year of publication on the topic, including articles published in English and Portuguese available in full. A qualitative assessment of the published content was carried out. In this study, the consequences of compromised kidney function and how it can directly impact the lives of sufferers were addressed. The fact that systems of the human body are compromised due to this disease makes it necessary for the dental surgeon to have training and knowledge applied in his routine care for these patients. It is worth noting that CKD can directly or indirectly affect the oral health of patients with it. This review also indicated some of the medications of choice for these patients in Dentistry and what dental care protocols should be like, taking into account the different stages in which the disease may be found and whether this patient is transplanted or not. |

Es una enfermedad provocada por la destrucción de la unidad funcional renal, llamada nefrona, y que tiene diferentes etapas hasta derivar en una insuficiencia renal total. Este trabajo tiene como objetivo dilucidar las particularidades relacionadas con la atención odontológica de personas con enfermedad renal crónica, en hemodiálisis y receptores de trasplante renal, además de dilucidar protocolos sobre la atención en el cuidado de estos pacientes en la rutina del consultorio odontológico. Para realizar esta revisión narrativa se utilizaron las bases de datos Scielo y Pubmed, con las siguientes palabras clave: “insuficiencia renal crónica” “odontología” y “manifestaciones orales” asociadas por el operador booleano “E”, así como sus homólogos en inglés. Los artículos fueron seleccionados sin limitar el año de publicación sobre el tema, incluidos artículos publicados en inglés y portugués disponibles en su totalidad. Se llevó a cabo una evaluación cualitativa del contenido publicado. En este estudio, se abordaron las consecuencias de la función renal comprometida y cómo puede afectar directamente la vida de quienes la padecen. El hecho de que los sistemas del cuerpo humano se vean comprometidos debido a esta enfermedad hace necesario que el cirujano dentista tenga capacitación y conocimientos aplicados en su atención rutinaria a estos pacientes. Vale la pena señalar que la ERC puede afectar directa o indirectamente la salud bucal de los pacientes que la padecen. Esta revisión también indicó algunos de los medicamentos de elección para estos pacientes en Odontología y cómo deben ser los protocolos de atención odontológica, teniendo en cuenta las diferentes etapas en las que se puede encontrar la enfermedad y si este paciente es trasplantado o no.A doença renal crônica (DRC) é um problema de saúde pública que vem se destacando no decorrer dos últimos anos. É uma doença causada pela destruição da unidade funcional renal, denominada néfron, e que possui diferentes estágios até que determine a falência renal total. Este trabalho tem como objetivo elucidar as particularidades relacionadas ao atendimento odontológico de pessoas com doença renal crônica, em hemodiálise e transplantadas renais, além de elucidar protocolos sobre os cuidados no atendimento desses pacientes na rotina do consultório odontológico. Para realizar esta revisão narrativa, foram utilizadas as bases de dados Scielo e Pubmed, com as seguintes palavras-chaves: “insuficiência renal crônica” “odontologia” e “manifestações orais” associadas pelo operador booleano “E”, bem com seus correspondentes em inglês. Foram selecionados artigos sem limitação de ano de publicação acerca da temática, incluindo artigos publicados em inglês e português disponíveis na íntegra. Uma avaliação qualitativa dos conteúdos publicados foi realizada. Neste estudo, foram abordadas as consequências de uma função renal comprometida e como ela pode impactar diretamente a vida dos portadores. O fato de sistemas do organismo humano serem comprometidos, em razão desta doença, torna necessário que o cirurgião-dentista tenha capacitação e conhecimentos aplicados em sua rotina de atendimentos sobre esses pacientes. É digno de nota que a DRC pode afetar diretamente ou indiretamente a saúde bucal dos pacientes portadores.
